Employee was a passenger on mantrip 45 traveling out of the mine at end of shift. The mantrip inadvertently struck a pot hole, located on the inner seam slope, in travel road injuring employee.
Employee states while employee was installing roof bolts in the Main North 2 travelway the roof bolter machine was bumped by a ram car causing injury to right hip. Restricted Duty.
Was spreading rock dust that was on travel way with drag attached to ram car. Was watching drag when a piece of T-Channel on rib hit me. Cause of injury T-channel loose on one end sticking out. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing 7/26/16
Employee was riding out in a man trip when man trip hit a hole in travel way causing employee to be bounced around in compartment. Cause of injury was an undetected hole in travel way. Incident did not involve any aspect of compliance with rules and regulations & PPE. Mine equipment involved & miner proficiency may be a factor. Started to miss work 2-9-16.
Employee was operating a Kubota on the travel road. The front tire caught a notch in the rib due to the employee glancing over in a x-cut while driving and the Kubota turned over on its side. The employee struck head on the roll over protection bar. Restricted Duty.
Employee was riding in mantrip #8 in the 10WHG travel road. The mantrip struck a pothole causing the employee to feel pain in lower back. Lost time injury.
Employee was operating a ram car and ran over a concrete block in the travel road causing pain to lower back.
Employee was working on a scoop that was separated at the middle located in the underground shop. A different scoop in the area trammed and bumped the rear end of the separated scoop causing it to move bumping the employee's left shoulder. Restricted Duty.
Employee states his hand was outside of the cab of the scoop, and when turning 3 of his fingers were caught between the cab of the scoop and the rear section of the scoop. Compliance with rules and regulations, involvement with mine equipment or mine systems, or protective items are not a factor. Job skills and miner' s proficiency may be a factor.
Employee was pulling an existing 90 foot utility pole with a digger derrick truck. The employee was rotating his boom to the drivers side of the truck when the weight started causing the passenger side outriggers to come off the ground. He then boomed down & winched off lowering the pole but it flipped the truck throwing the employee off.
Employee was struck on his left side by a power vehicle.
EE was operating a shuttle car with his hand on the post of the car when his ring finger on his right hand became caught between the canopy and the frame of the car. Medical treatment injury.

EMPLOYEE WAS MAKING REPAIRS TO THE BELT WHEN HE SLIPPED AND GRABBED HOLD OF THE BELT. THE BELT THEN PULLED HIS RIGHT RING FINGER WITH GLOVE INTO THE PULLEY CUTTING OFF THE END OF THE FINGER.

EE was scooping around the feeder area, when the bucket of his scoop caught the bat wing and as he continued to back up, the bucket dropped down, causing EE to jam his head into the canopy. Reportable lost time injury as of 9/30/10 due to surgery.
EE was injured, when the mantrip he was riding to the outside of the mine, rubbed the side of the rib. EE's left foot, which was outside the mantrip, was caught between the rib and the mantrip. Lost time injury.

EE was driving a personnel vehicle when he lost the brakes and ran into the back of a ram car.
Employee was in a ramcar when a piece of T channel hanging down came in the cab and struck employee in the right leg and thigh and left index finger, causing laceration. Lost time injury.
Employee was driving a golf cart when she stated that the steering wheel turned 90 degrees, flipping the cart and pinning her underneath.
EE was unloading belt material out of the back of a ramcar, when he failed to acknowledge an approaching ramcar and was struck from behind.
Employee ran over a rock that was not visible, which caused the steering wheel to turn sharply to the left. Employee's right hand was twisted to the left.

Employee was driving a Getman when he hit the rib, causing the rib to break and strike him on the neck and shoulder. Lost time injury.
Employee was driving a shuttle car, when curtain hung up on the cab, bringing the curtain inside the cab. Employee stopped the car. As he pushed the curtain out of the cab, his foot came off the brake, causing the car to roll backwards, pinning his arm between the canopy and the rib. Lost time injury.
Employee had attempted to flag the driver of the belt sled to stop so he could hook up the sled. Driver did not see employee and his leg was caught between the ramcar and sled. Restricted duty reportable 12/7/08.
Employee was injured while he was loaidng calcium choloride on a pv when a ramcar struck a parked mantrip which struck the employee.

Employee had his right hand on the ramcar tailgate. As they were moving the bed back in, the tailgate jack cylinder pinched his right index finger.
Employee was operating GC72 and was moving out of the crosscut when the front wheel hit a rut, causing the golf cart to go backwards down the underpass until it hit the opposite side of the hill and turned over on top of employee and passenger. 6/25/08 He saw physician, x-rays revealed fracture to right wrist. Restricted duty injury.

A mantrip and maintenance personnel vehicle were passing in a travelway when the back of the mantrip hit the back of the personnel vehicle. Employee's right leg was hanging outside the mantrip and was struck.
Employee was on the ground helping the scoop turn over a subbase for the belt drive when the bucket caused the subbase to shift, knocking employee down.
Employee was operating RC26 in the Flannigan 1WHG. He ran into some screen wire that was hanging down. The screen wire struck him in the head.
While passing the air ride PV, the rear of PV caught the mantrip seat & bent the seat back into EE's shins.

Employee was operating an end loader, pushing a ramcar when the bucket of the end loader caught a 2x4 that was pinned to the roof, causing the 2x4 to break and come inside the cab, striking his face and neck.
Employee took parts to the Galatia North Portal. While using the forklift to put the parts on the cage, he tried to push the door open further while still on the forklift. The forklift rolled back and caught employee's left arm between the forklift canopy and door frame.
