IE claims that the tractor of IE's truck began to bounce causing IE to bounce in the seat. As a result IE suffered from back pain. Incident became reportable on 10/22/2021 when the IE was placed on light duty.

Showing all 11IE assisting in changing dump ropes at the dragline. Coworker operating the tool carrier was hoisting the attached dump rope to reposition when a loop in the drag rope flipped over causing the cable/rope to strike the IE in the right ankle (who was walking away at the time of the incident) which caused the ankle to roll resulting in an ankle sprain.
Truck operator traveled over an area that had been spot watered on a 3% grade. The truck slid sideways and then contacted a dry area of road. The right front tire deflated and the truck rolled over onto it's right side.
IE was operating a belly dump truck. IE rested his arm on the window track. He hit a bump in the haul road and for an unknown reason jerked his arm from the window track. When he jerked his arm back his hand struck the window latch resulting in a laceration to his left thumb. The laceration required 6 stitches.
The scaper driven by EE was traveling at a high rate of speed. (8th gear) A water truck driven by Other EE had stopped in the road to let the motor grader make his turn. The scraper made contact with the water truck on the right rear corner. The damage estmate was $44,000 The operator stated that he was OK at the time of the accident, but went to the doctor the next day.
Directing forklift operator to get steel on forks to unload 18-wheeler flat bed. While taking the top steel off, the bottom beam moved making contact with employee's right lower leg. Employee fell off bed causing injury to right heel.
Employees advancing trailing cable. After advancing, he went to rear of skidder, removed cable and motioned operator to move forward. When he dropped the cable, it got caught on a hook. As skidder traveled forward tension on the cable increased and cable struck ee in the lower chest and flipped him to the ground. X-rays show fractured ribs and a fracture in #4 Metacarpal.

EE WAS HAULING COAL TO THE "D" STOCKPILE IN A UNIT RIG 300 TON COAL HAULER.
