Showing all 11EE STATED HE WAS DRIVING A DIESEL PERSONNEL CARRIER ONT HE 2ND SUPPLY RD. HE LOOKED AT THE HIGH VOLTAGE CABLE AND RAN INTO LONGWALL PANS PARKED ON THE SIDE OF THE ROAD. HE DID NOT SEE THEM. HE STRUCK HIS LEFT LEG AGAINST THE DASHBOARD AND RECEIVED A LACERATION ABOUT 4" BELOW HIS KNEE. HE MISSED WORK ON 12-17-02 AND RETURNED TO WORK 12-18-02.
EMPLOYEE REPORTED: OPERATING A GOLF CART, WHEN BACKING THE CART, HIS JACKER BECAME ENTANGLED ON A PIECE OF METAL. THIS CAUSED HIS BODY TO BE TWISTED, RESULTING IN A POSSIBLE STRAIN TO HIS LOW BACK. HE CONTINUED TO WORK ALL SCHEDULED SHIFTS UNTIL 1/24/03. ON THAT DAY STARTED MISSED SCHEDULED WORK DAYS AND BECAME A REPORTABLE INJURY.
EMPLOYEE REPORTED: HE AND A COWORKER WERE OPERATING SHIELD HAULERS TRANSPORTING THE "MUTT" FROM THE 6TH SOUTH TO THE LONG WALL RECOVERY CHUTES. BOTH OPERATORS HAD DUCK BILLS UNDER THE MUTT CAR RYING THE MACHINE ALONG MINE TRAVELWAY. THE COWORKER TRAMMED FORWARD PULLING THE MUTT FROM THE VICTIM'S DUCK BILL, THIS CAUSED HIS MACHINE TO BOUNCE RESULTING IN HIS HEAD STRIKING THE CANOPY.
EE REPORTED: HE & A COWORKER USING SHIELD HAULERS WERE TRANSPORTING A COAL FEEDER FROM A WORKING SECTION TO THE PORTAL ALONG THE MINE SUPPLY ENTRY. THE FEEDER SLID OFF THE VICTIM'S SHIELD HAUL ER FORKS, THIS ALLOWED THE MACHINE TO BOUNCE CAUSING HIS HEAD TO STRIKE THE TOP OF THE SHIELD HAULER CANOPY ROOF. HE IS EXPERIENCING PAIN & STIFFNESS TO HIS NECK.
EE STATED HE WAS SITTING IN HIS 777 HAUL TRUCK BEING LOADED BY 992G ENDLOADER WHEN A LARGE ROCK WAS DROPPED INTO THE BED CAUSING CONSIDERABLE JARRING AND VIBRATION RESULTING IN PAIN TO LOW BAC K.
EE STATED OPERATING A MINE PICKUP TRUCK ALONG THE MINE TRAVELWAY. BUMPED HIS HEAD AGAINST AN OBJECT ALONG THE MINE ROOF WHEN ADJACENT TO X CUT 54 OF THE 1ST. MAIN SOUTH SUPPLY ENTRY. UNSURE OF WHAT HE STRUCK.
WHILE TRAMMING RAM CAR HE RAN OVER A PIECE OF ROCK ON THE MINE FLOOR CAUSING HIM TO BE THROWN UPIN THE OPERATOR'S COMPARTMENT. HE LANDED ON HIS SELF RESCUER CAUSING A CONTUSION TO LOWER BACK.
EMPLOYEE REPORTED: TRAMMING RAM CAR ALONG THE 9 LEFT TRAVELWAY, RAN OVER A ROCK, HE WAS THROWN UP AGAINST THE CANOPY ROOF RESULTING IN INTERNALPAINS TO HIS NECK.
VICTIM WAS ATTEMPTING TO HOOK UP A MINER BOOSTER PUMP TO THE FRONT OF THE FORKLIFT OPERATED BY ANOTHER EE. EE ACCIDENTLY HIT THE ARTICUATION LEVER AND INJURED WAS CAUGHT BETWEEN THE FORKS AND THE PUMP RESULTING IN A FRACTURED PELVIS.
EMPLOYEE STATES HE WAS DEADHEADING 950 ENDLOADER/FORKLIFT TO DRILL SITE IN THE PIT TO USE ON DRILL REPAIR. HE TESTED BRAKES AT TOP OF RAMP AND THEY WERE WORKING, HOWEVER AS HE TRAVELED DOWN RAMP THE BRAKES FAILED. HE TURNED THE ENDLOADER INTO SPIL ON LEFT SIDE OF RAMP, CAUSING THE ENDLOADER TO TURN ON THE RIGHT SIDE.
