Employee was operating a 2 man diesel, His vision became blurred, before he could stop he bumped into a supply trailer in the roadway.
Employee stated that he was loading belt rollers onto the forks of a forklift. As he was loading one of the rollers he failed to lift it high enough to clear the end of the forks and injured his finger. He went to the doctor and was treated for a bruise.
Employee was walking beside a de-energized ram car. Went through a clear curtain the ram car was parked under. The operator of the ram car did not realize he had not cleared the ram car, and energized the machine engaged the steering swung the bed and caught him between the rib and the bed fracturing his right femur.
Employee stated that he was riding on a golf cart with his right hand resting on the golf cart fender. When the golf cart passed through a back-up curtain his right hand was caught between the golf cart and a ram car that was parked there. This smashed his right hand causing a laceration on his small finger, that required sutures.
Employee states he was positioning forks of mini trac when another employee picked up the end and caught his finger between the mast and the fork.
Submitted under protest to terminate citation #8514567. Employee stated that he was hauling material in scoop bucket when he ran over debris in road causing him to hit his head on canopy. Operator is currently investigating the claim and reserves the right to amend and/or correct this report.
Stated: Employee was examining the 4C belt line. He was driving a golf cart. As he was driving through an area where clearance between rib and belt structure was limited, one of his front tires hit something. It caused the steering wheel to jerk and threw his left arm into the belt structure. A bone in his arm just up from his wrist was fractured.
Stated: Employee was steering a 4 man ride being towed to bottom. A pinner steel was lying on the ride. As they turned a corner one end of the steel became lodged in something causing the other end to come around and strike the back of his head. He worked until 1/18/13 when his doctor placed him on complete mental rest.
Stated: Employee was driving golf cart toward bottom when his steering malfunctioned. His golf cart ran into the rib and he hit his head on the steering wheel and his left leg on the dashboard. He went to the ortho on 10/22/2012 and was diagnosed with hairline fractures to his fibula. He is to remain off work until healed.
Submitted under protest to abate citation #8511176. EE struck head on a scoop canopy on 8/21/12. Missed no work until 9/14/12 diagnosed with a normal CT scan and released to RTW with no lite days. Did not mention being injured until 10/11/12, Is currently investigating the claim and reserve the right to amend and/or correct this report.
Stated: Employee was driving golf cart on travel road and ran over a butt off a timber. The butt lodged under the golf cart and in the road, causing the golf cart to swerve sideways and wreck. It threw him into a gob pile in a XC. He went to the ER and was diagnosed with left shoulder pain, low back pain and fracture of first left rib.
Stated: the employee was riding in man trip with curtain covering wet seat. Part of the curtain was hanging out and caught on a bundle of bolts around a fall area. The curtain pulled and twisted his knee. Diagnosed with degenerative knee and meniscal tear, no surgery will benefit. Due to waiting on MRI results to determine injury/illness classification, this report is filed late.
Stated: Employee was tramming a ram car down #9 entry (#5 unit) & hit a hole in the road and his hard hat struck against the canopy. He felt pain in the right side of neck. He was treated by a chiropractor April 30, 2010 for neck pain and did not miss any scheduled work.
Attempting to lower batter off of joy ram car, he failed to have car properly positioned. Car was titled and battery slid off the rear of car. This allowed the car to bounce causing his head to strike against the canopy of the car. On the following Monday, on the 2nd shift at mid shift stated his neck was sore and he wanted to be taken from the mine.
EE reported: Cleaning alongside the 5A belt at the 5B dump location. A piece of fire clay came over the head roller, struck the dump backboard, breaking up and a small piece ricocheted, striking his cheek and broke his upper tooth.
Operating ram car, tramming along haulage road, ran over fallen draw rock, caused her to strike hardhat against the machine's canopy. Continued working until 11/10/08, due to neck pain. Previous neck injury in 2004 and treated for over 1 year by two doctors. Nov, 2007 involved in car accident, taking meds for neck pain up to 9/27/08. Mine charging event as a work related illness.
Operating Getman tractor backing the machine. When attempting to repostion body inside compartment(turning to improve vision)placed his right hand onto a guard for the stop block. His finger were wrapped around the edge of the guard. When the machine articulated together, his right little finger was caught by the stop block, resulting in a fracture to the 1st joint of the finger.
EE reported: In ram car charge station, operating ram car performing task of changing battery. Prior to articulating frame of car, he engaged the battery release hook, this allowed battery to suddenly roll off the car frame. The sudden release of the battery resulted in the car frame quickly raising. This jarred his low back resulting in internal pain to lumbar region
Crew had moved section power box. EE & coworker were attempting to reconnect high voltage power cable plug to rear of sub. Positioned behind sub was high voltage cable storage tub. He was standing between sub & storage tub. A scoop traveling in x-cut behind tub struck rear corner of tub, slewing it sideways pushing front of tub forward catching his ankle against rear of power box.
Employee reported: Attempting to clean water sprays on inner belt spray for the FCT. Twice turned off belt, tapped on spray bar with hammer. After 2nd attempt, with belt running, again tapped on bar. Hammer was pulled under a roller, pullng is arm under roller also, immediately turned off belt.
EE reported: Operating a DBT scoop cleaning gob from crosscut. Attempting to back-drag material, tilted bucket and backed machine. Toe of bucket dropped off a 12" ledge, machine dropped suddenly causing EE's head to strike roof of canopy.
Employee reported: Over the last few weeks of performing his normal work duties as a Supply tractor operator (Getman Operator) has experienced pain and soreness to low back region. This is the same location where he has had three previous disc surgeries. There is no specific event that has caused this new problem. The mine is charging this claim as a work related illness.
EE reported: Operating a golf cart along the Main North travelway entry. Passed under the No. 1 unit intake overcast as he was coming up the incline on the outby end. Struck his head against the outby brow of overcast, resulting in possible strain to his neck.
EMPLOYEE REPORTED: OPERATING GOLF CART ALONG MINE TRAVELWAY. HE RAN OVER A ROCK ON THE MINE FLOOR, THE STEERING WHEEL TURNED QUICKLY CONTACTING HIS LITTLE FINGER LEFT HAND. A ROUGH SPOT ON THE STEERING WHEEL CONTACTED HIS FINGER RESULTING IN A SMALL LACERATION. THE INJURY REQUIRED SUTURES TO CLOSE THE WOUND.
EMPLOYEE REPORTED: WORKING AS THE "DOLLY WATCHER" ALONG THE FCT BELT. AS THE FCT WAS TRAMMING ALONG THE BELT, THE MACHINE'S POWER CABLE STRUCK HIS RIGHT FOOT PUSHING IT AGAINST THE FRAME OF THE LOW BELT STRUCTURE. THE EMPLOYEE REPORTED SORENESS TO THE HEEL OF HIS FOOT.
THE COAL DISCHARGE HAD BECOME STOPPED UP WITH LOOSE MATERIAL. THE MACHINE'S WHISPER SWITCH HAD BEEN DEPRESSED BY THE MATERIAL DEACTIVATING THE CONTROLS. AFTER CLEARING MATERIAL, HE STEPPED ONTO THE GUIDE RAIL FOR MACHINE'S DOLLY. WITH THE WHISPER SWITCH IN NEUTRAL, THE OPERATOR TRIED TO RESTART MACHINE. WHEN IT MOVED, IT CAUGHT HIS FEET BETWEEN RAIL WIPER BAR AND DOLLY RAIL.
EE WAS RIDING AS A PASSENGER IN AN AL LEE DIESEL 2-MAN RIDE. OPERATOR WAS BACKING THE VEHICLE INTO A CROSSCUT. EE WAS REPOSITIONING HIS BODY. HIS FOOT WAS PLACED ON THE FIRE SUPPRESSION TANK FOR LEVERAGE TO HELP LIFT HIS BODY. EE'S FOOT SLIPPED OFF THE TANK, CAUGHT UNDER THE RIDE AND SLIGHTLY DRUG ALONG THE MINE FLOOR, TWISTING HIS ANKLE. RESULTED IN A SIMPLE FRACTURE: ANKLE.
EMPLOYEE REPORTED: HELPING CO-WORKERS LOAD ROOF FALL OFF OF FCT IN NO.UNIT BELT ENTRY. THE CREW HAD A NYLON STRAP WRAPPED AROUND A PIECE OF ROCK ATTEMPTING TO DRAG THE ROCK WITH A SCOOP. WHEN PRESSURE WAS APPLIED TO THE STRAP, FOREIGN MATERIAL POPPED FROM THE STRAP STRIKING HIS RT. EYELID.
EE REPORTED: RIDING AS A PASSENGER IN A 14 MAN DIESEL MANTRIP, THE MANTRIP BECAME HUNG UP WHEN THE OPERATOR PULLED INTO A CROSSCUT ALONG 2-A SUPPLY ROAD. ANOTHER WORKER, USING ANOTHER D IESEL VEHICLE, TRIED TO PUSH THE STUCK VEHICLE TO FREE IT. THE SECOND VEHICLE'S BUMPER CAME OVER THE SIDE FRAME OF THE 14 MAN MANTRIP RESULTINGIN THE VICTIM'S FOOT BEING STRUCK BY BUMPER.
AT APPROX. 7:30 PM, ON SAT ONE CREW WAS WORKING IN THE SLOPE AT 8 FT AND ONE CREW WAS WORKING AT THE PORTAL OF THE SLOPE INSTALLING THE KNUCKLE ROLLER ASSEMBLY WHILE POSITIONING THE ASSEMBLY A T THE PORTAL THE ASSEMBLY, WHICH IS NOT TIED OFF, STARTED SLIDING DOWN THE SLOPE. IN SLOPE SUPERVISOR EE WAS UNABLE TO SEEK SHELTER, FOR HE WAS RIDING THE MUCK CAR TO THE SURFACE. THE ASSEMBL
WHILE HOOKING CHAIN TO SCOOP, EE WAS STANDING INSCOOP BUCKET, SCOOP BUCKET FELL TO MINE FLOOR CAUSING EE TO COME DOWN HARD ON ANKLE.
EMPLOYEE WAS CHANGING OUT BATTERIES ON A RAM CAR - HE PLACED HIS HAND ON TOP OF THE CAR'S TIRE & THE RAM CAR MOVED FORWARD WHICH CAUSED HIS HAND TO COME IN CONTACT WITH THE FENDER OF THE WHEEL
MASHED FINGERS BETWEEN RUB RAIL AND MINE FLOOR WHILE RIDING ON A MANBUS.
EE WAS DRIVING MANTRIP & HIT HIS HEAD ON A RAIL BAR
EMPLOYEE STATED: TRAMMING SCOOP ALONG MINE TRACK, RAN OVER BLOCK OF WOOD, STRUCK HEAD AGAINST MINE ROOF, JAMMING NECK.
EE STATED: OPERATING LOCOMOTIVE ALONG THE MINE TRACK, STRUCK HIS HEAD AGAINST A RAIL BAR, CONTINUED TO WORK UNTIL 9-8-2000.
EE STATED: OPERATING SUPPLY JEEP, BUMPED HIGH VOLTAGE CABLE WITH MACHINE'S CANOPY, CABLE CAME OFF HANGERS, FELL, BUMPING HIS HEAD.
EMP STATED: REMOVING ROCK FROM FEEDER, FLIGHT CAUGHT ROCK, ROCK MOVED SUDDENLY CATCHING HIS FINGER AGAINST FEEDER FRAME.
EE STATED: HE AND CO-WORKER HAD BROKEN UP ROCK IN FEEDER. WHEN FEEDER WAS RESTARTED, PIECE OF ROCK RAISED UP CATCHING HIS HAND AGAINST FRAME OF FEEDER.
EMPLOYEE STATED: OPERATING MOBILE BRIDGE, CANOPYCAUGHT ON ROOF BOLT, THE CANOPY RAISED THEN DROPPED SUDDENLY, THE ROOF OF CANOPY STRUCK HIS HEAD.
