EE backed up to coal pile at Quincy Dock to dump a load of coal. EE raised EE's bed about 2 stages and the tailgate did not release. At that time EE lowered the bed but the dump bed was leaning too much and the truck turned over - truck and trailer both damaged, EE sustained 2 broken ribs and a bruised leg.

Kanawha River Terminals · Fall onto or against objects

Employee was standing inside locomotive preparing to dismount when a faulty air valve on one of the railcars caused the train to enter emergency mode and stop. This unexpected stopping motion caused the employee to fall inside the locomotive and hit his head. Doctor put employee off for two days.

Driver was instructed what stockpile to dump was in. When driver backed into position to dump. He informed loader op. He was not level enough to dump. Driver pulled forward a few feet and loader op. told him to stop there and dump and the loader op. would spot him (put bucket beside trailer) loader op. alledgedly failed to spot truck and it overturned.

EE got out of his truck to clean his tailgate and did not set his parking brake. The truck began to roll, ee tried to get back in the truck, lost his balance and fell beside the truck which ended up on his lower legs. He had severe bruising and contusions to this area esp right leg.

Employee was loading a coal hopper with 988F model front end loader. When employee entered the stockpile to retrieve another load, there was a sudden jolt caused by the loader, resulting in a low back strain. According to the WCD Report the probable length of the injury is more than 4 weeks.

TRUCK "A" WAS DUMPING HIS LOAD WHEN TRUCK "B" BACKED IN BESIDE HIM. TRUCK A'S TAILGATE FROZE SHUT AND HIS LOAD DID NOT COME OUT, HE PULLED FORWARD, THE TRAILOR ROCKED AND THEN THE BOTTOM JACK PIN BROKE AND THE TRAILOR FELL OVER ONTO TRUCK B'S DRIVERS SIDE. THE TRAILOR TRAPPED INJUREDS FOOT ONTO THE FLOOR BOARD.
