Employee was helping to move the power on the longwall section. The monorail became hung up and the injured employee was feeling a joint of it looking for a rough spot. In the process of feeling the monorail it broke free and smashed the employees' right hand. This caused two of the employees fingertips to be mashed.

Employee was helping unload supplies on the 6 West section at the supply doors. They had hooked up the pull chain to a pilot of bulk dust and the scoop so they could pull the dust into the scoop. In the process of pulling the dust into the scoop, the pull chain broke and came back and hit the employee who was standing beside the flatcar, in the left hand and leg.

Employee was scooping gob out of a break on the long wall when a cable bolt flipped up out of the gob striking the employee in the back of the head causing his forehead to strike the value chest in the operators compartment.

Employee was operating scoop near the bottom of the slope when he lost brakes suddenly and struck a concrete pad stopping abruptly. Employee continued to work until 4/24/15 when he was taken off work under Doctors order. The accident was deemed reportable by Workers Comp. on May 12, 2015.

Employee was operating a 9 man Brookville track mounted mantrip traveling outby on 5 west. The employee came around a curve when the mantrip de-railed causing the employee to strike his left shoulder on the frame of the mantrip.

Employee was operating a 740 Caterpillar lizard truck on the refuse dump when he slid into a rut causing the operator to be thrown to the left then back to the right injuring his lower back and right thigh. Note: Employee did not start missing work until 9/26/2014.

Employee was tramming scoop across the tracks at the Parallel Mains supply door to scoop trash up from the trash hole. The scoop ran upon a crib block and the crib kneeled over causing the employee to bounce and strike his head against the scoop canopy.

The employee was operating a shuttle car on the South Mains Section. The operator was returning from the miner in eight right. As he passed through the #6 intersection, the shuttle car ran over a hole in the mine bottom bouncing the employee into the canopy.

Employee was shoveling on off-side of belt on 17 HG B-Belt, approximately 20 feet outby tailpiece. A slate bar was laying on top of the tailpiece. It is believed that a rock knocked the slate bar onto the belt. The slate bar struck the V scrapper and bounced off striking the employee in the hand causing a laceration.

Employee had knocked some stoppings with the scoop. A few of the blocks from the stopping were laying on top of the scoop canopy. When he trammed the scoop back across the section, he went under the bolter cable that pushed the block off of the scoop canopy. The block fell from the canopy and struck the employee in his left middle finger.

EE was trying to retrieve some rope hangers from on top of feeder. The feeder was sitting on uneven mine floor and the feeder shifted back smashing his right toe.

Employee was operating a Fairchild scoop and swiveled the front section of the scoop to the right. This caused the rear section to spring the entrance door on to employee's left hand.

Employee driving company truck on way to assigned shift. Driving downhill, said brakes felt "hard", lost control of vehicle, went into skid, hit fan support at site.

Employee alleged he was operating a shuttle car coming from miner to feeder when he hit a hole and hurt his back. *Filed under protest due to part 50 audit.

Employee was operating an end-loader to move a continuous miner boom when the rear wheels of the end-loader came off the ground and then went back on the ground.

Employee was riding in the #3 4-person mantrip traveling outby on 12 headgate's track. The mantrip operator noticed a mantrip parked on the track and applied the brakes causing him to slide into a parked mantrip. The collision caused EE to fracture 3 ribs & dislocate his right shoulder.

Employee was roving client property in company vehicle, hit black ice on roadway and lost control, vehicle went over an embankment.

Employee was riding in a locomotive that was approaching another locomotive. He though they were going to collide (they did not) and he jumped out of the locomotive.

Employee's right ankle got wrapped up in a piece of 2" waterline that was hooked to the section pumping station. As it was being pulled up to the face by a scoop.

Employee was riding mantrip. Mantrip came off the tracks and crushed into the rib.

VICTIM WAS COUPLING MOTOR AND FLAT CAR. MOTORMAN DIDN'T KNOW INJURED WAS BETWEEN MOTOR AND CAR. INJURED HIS LEFT LEG BETWEEN FLATCAR AND MOTOR.

Victim was walking beside track, being followed by a mantrip. victim side stepped into the path of the mantrip and the mantrip struck his left leg.

Victim was operating a mantrip when matrip wrecked striking rib.

Ee was riding mantrip when mantrip struck truss bolts stacked by tracks. Truss bolts struck ee on right arm.

INJURED WAS OPERATING A TRACK MOUNTED MANTRIP. WHEN HE HIT A ROCKDUSTER CAUSING HIS LEFT HAND TO GET CAUGHT BETWEEN THE FRAME AND BRAKE HANDLE. THIS RESULTED IN A FRACTURE TO HIS LT. HAND.

INJURED WAS HELPING REMOVE ROCK OFF OF CONTINUOUS MINER WHEN THE SCOOP BUCKET HIT HIS RIGHT LEG. THE PERSON CONTINUED TO WORK UNTIL 9-27-02.

PERSON WAS OPERATING A BROOKVILLE TRACK-MOUNTED MANTRIP WHEN HE STRUCK A FLAT CAR CAUSING HIM TO INJURE HIS RIBS AND BACK.

PERSON WA SOPERATING A MANTRIP WHEN HIS LEG GOT CAUGHT ON A BELT ROLLER LYING BESIDE THE TRACK INJURING HIS RIGHT KNEE.

INJURED WAS HAULING A 480 VOLT PUMP IN A TRACK MOUNTED MANTRIP WHEN IT STOPPED. THE PUMP TURNED OVER STRIKING HIS LEFT ANKLE.
