Showing all 27A haul truck driver claims EE was jarred while EE's truck was being loaded. EE continued to work until 8/8/24, at which time EE was seen by a doctor and not allowed to return to work.
Employee operating a scraper and he ran over a frozen chunk of material. The impact "Jarred" him in the seat and he experienced pain and general discomfort in his left testicle. He was examined after shift at the clinic and again before his next scheduled shift and was unable to report for next scheduled shift.
Employee drives a 240 ton coal hauler and while being loaded, when the first bucket of coal was dropped into the truck, the employee experienced pain in neck and back. EE was examined by a Medical Dr. and has begun Physical therapy. Neither time nor wages have been lost.
Employee was operating a coal hauler and drove off the right side of the road, traveled over a small pile of dirt, back up the side of the road and over a large berm. As a result the employee received a laceration and was taken to physician for medical treatment.
Employee was assigned duties operating a tractor trailor for coal hauling. He first reported pain in his lower back on April 19, 2004. His back has since became more aggravated. He went to a physician for treatment and was unable to return to work for his next scheduled shift.
Employee was hauling coal and he drove the hauler over uneven terrain, causing jarring in his lower back. He continued working, however the pain continued and he sought chiropractic treatment to help relieve the pain. To date neither time nor wages have been lost.
While operating a front end loader, the seat bottomed out when he hit a bump. He experienced pain in his hip, ribs, and lower back. The employee sought chiropractic treatment to alleviate the pain. Neither time nor wages have been lost. Employee did not report incident until 12/20/2006.
Employee was laying topsoil with a scraper and ran over a rock with the front tire. He experienced pain in his neck and upper back. He sought chiropractic treatment to help relieve the pain. He continues to work, and to date no time has been lost.
Employee was operating a scraper and tried to hook up to a scraper in front of him, the push block caught on top of the front machine. When they seperated, his machine dropped to the ground, and he experienced pain in his back. He was diagnosed with a compression fracture of a thoracic vertebrae. He was unable to work his next scheduled shift.

EE WAS POSITIONING TRAILING CABLE FOR PLANNED DRAGLINE MOVE. PULLING ON CABLE ACROSS A SLOPE OF ABOUT 4 TO 1 GRADE. 1 FRT TIRE OF THE REELER TRAVELING INTO SLIGHT DEPRESSION & LOADER ROLLED ONTO ITS SIDE. THERE WAS ABOUT 1200FT OF 12,500 VOLT CABLE ON REEL. EE WAS USING SEAT BELT,BUT STRUCK HIS MOUTH & CHEECK ON SOME OBJECT & REQ'D SUTURES IN HIS LIP
Employee was moving nine loaded rail cars down a grade and the brakes on the shuttle he was driving failed to activate when applied. The cars contacted a string of nine parked loaded rail cars. Employee was medically evaluated and began Physical Therapy on 09/09/2005. He has continued working. On 2/1/06 he had surgery on his shoulder to relieve the pain.
Employee was operating a water wagon on a haul road with a 70' top when a hydraulic line failed causing loss of steering. The water wagin went through the barrow ditch and onto natural terrain. Employee experienced pain in his back going through ditch. He began chiropractic treatment on 08/09/05. No time has been lost.
Employee was hauling topsoil with a 657-E Caterpillar scraper. He hit a bump and experienced pain in his low back when the cushion hitch malfunctioned. On 06/05/05 he received an injection for pain, and on 06/14/2005 he began physical therapy.
On 12/30/2004 the employee reported ever increasing pain in his shoulders and attributes it to his duties as an over the road truck driver(rolling tarps, shifting, etc). On March 30, 2005 he reported that he began receiving physical therapy to relieve the pain. Neither time nor wages have been lost
Employee was a passenger in a Euclid Ch-120 haul truck riding in the jump seat. The loaded truck stopped to dump and when the brake was set the tractor frame failed breaking nearly completely through on both sides. The cab fell forward contacting the ground with significant impact. Employee struck head on air conditioner and his lower leg on the passenger door.
Driving into pit, the employee's truck jack-knifed on wet road and came to a sudden stop, jarring his neck and back. Examined that day, returned to work with no restrictions. On November 17, 2004 employee notified company that he was receiving physical therapy for pain management. To date has missed no time and performed no restricted work.
While operating a water wagon the employee drove over a bump in the road, the air seat bottomed out and did not return to the original position. He reported pain in his lower back from the incident. He is currently receiving treatment from a chiropractor, but is working his regular job title without restrictions. Neither time nor wages have been lost.
THE EE WAS HAULING OVERBURDEN IN A 100-TON END DUMP. HE WAS UNAWARE THE PAD HAD RECENTLY BEEN WATERED AND WAS ICY. WHEN HE DROVE ON THE DUMP PAD, THE TRUCK SLID ON THE ICE. WHEN THE TRUCK W ENT OFF THE ICE ONTO DRY MATERIAL, THE EE EXPERIENCED SEVERE PAIN IN HIS NECK. HE FINISHED THAT SHIFT AND THE NEXT TWO SHIFTS BEFORE SEEKING MEDICAL TREATMENT. AFTER EXAM, UNABLE TO RETURN.
THE EMPLOYEE WAS TEMPORARILY ASSIGNED DUTIES DRIVING A 160 TON DART COAL HAULER. HE NOTICED PAIN IN LOWER BACK AND LEFT HIP WHICH GRADUALLY INCREASED AS HE CONTINUED DRIVING THE HAUL TRUCK. HE SOUGHT CHIROPRACTIC TREATMENT AND WAS DIAGNOSED WITH LUMBAR AND PELVIC SEGMENTAL DYSFUNCTIONS, WHICH HE ATTRIBUTES TO REPETITIVE TRAUMA FROM OPERATING HEAVY EQUIPMENT.
THE EMPLOYEE INFORMED US HE WAS HAVING TOO MUCH BACK PAIN TO REPORT FOR HIS AFTERNOON SHIFT ON 03/18/03. HE RELATES IT TO BEING JARRED PERIODICALLY WHILE HAULING SCORIA ROCK WITH A 657 SCRAPER ON 03/17/03. HIS NORMAL JOB IS SCRAPER OPERATOR, BUT DUE TO WEATHER CONDITIONS, HE SPENT MOST OF HIS SHIFT HAULING SCORIA. HE IS UNABLE TO RELATE HIS BACK PAIN TO ANY SPECIFIC TIME, PLACE OR
EMPLOYEE REPORTED TO THE COMPANY ON JULY 23, THAT HE WAS HAVING BACK PAIN AND THAT IT WAS RELATED TO THE CONT SITTING AS AN EQUIP OPERATOR AND MOST CLOSELY RELATED TO WATER TRUCK OPERATION ON OR ABOUT JULY 3, 02. ON JULY 24TH HIS BACK CONDITION WORSENED AND HE WAS UNABLE TO WORK THAT SHIFT. HE HAS BEEN OFF SINCE.

HE OPERATED COAL TRUCK STARTING THE WEEK OF JUNE 17 AND HIS BACK STARTED BOTHERING HIM. ON JUNE 28 HE RAN A TRUCK THAT WAS RECENTLY RETROFITTED TO HAUL LARGER LOADS. HE SAID IT WAS ROUGHER RIDING AND PRODUCED A SIDE-TO-SIDE MOTION. HIS BACK WAS VERY PAINFUL SAW DR
EE WAS OPERATING A 657 SCRAPER ON JUNE 17 & 18, 2002. HE IS A QUALIFIED OPERATOR HOWEVER HAD BEEN ASSIGNED DRAGLINE DUTIES THE PAST SEVERAL YEARS. RECENT REDUCTION IN FORCE AT THE MINE CAUSED A CHANGE IN WORK ASSIGNMENT. DURING THE SHIFT OF6-18, HE STATED SLICK CONDITIONS AND THE JARRINGOF THE SCRAPER CAUSED NECK PAIN. HE SAW A DR. ON6-20 & HAS BEEN OFF WORK SINCE THAT TIME.
EE WAS OPERATING A 657 CATERPILLAR SCRAPER HAULING TOPSOIL TO A STORAGE PILE. WHILE TRAVELING W/A LOAD, HE HIT A SLICK SPOT IN THE HAUL ROAD. SCRAPER SLID SIDEWAYS TIL IT HIT DRY MATERIAL. WHE N IT STRUCK DRY MATERIAL, THE SUDDEN TRANSFER OFENERGY PRODUCED A JERKING MOTION TO UPPER BODY OF OPERATOR RESULTING IN BACK PAIN. PAIN GOT WORSE & HE WAS UNABLE TO WORK STARTING 2/18/02.
EE IS NORMALLY ASSIGNED TO DRAGLINE OPERATOR/OILER HE WAS TEMP ASSIGNED DUTY OF WHEEL LOADER OPERATOR. AFTER 3 SHIFTS HE EXPERIENCED PAIN IN HIS NECK & NUMBNESS IN HIS LEFT HAND. THIS MAY BE RELATED TO A PRIOR INJURY & WAS AGGRAVATED BY THE BOUNCING MOTION OF THE LOADER. EE IS A QUALIFIED LOADER OPERATOR
