Employee was working in the paste tailing basement using a skid steer loader to clean up. He reached over to pick some trash when he felt a sharp pain in the back of his right leg.

Employee was starting a HCL burner when he looked into the flame eye and he received burns to his face.

Employee was removing trash from a floor sump grating when she stepped out of the area and slipped and fell causing her to bruise her knee.

Employee scratched a 6010 weld slag with a welding rod. Small piece of slag entered his left bottom eye lid.

Employee was assisting in repositioning of trailer mounted welding machine in order to attach to truck hitch so that welding machine could be moved to Pronto Constructors yard.

Employee was driving a forklift down a road when he hit a pothole causing a strain to his right side abdomen.

Employee was removing lockout locks from a column cell when he accidentally got acid in his boots, he didn't notice it at first until later he realized that his foot was burning and he was treated for burns on his foot

Employee was exposed to CL2 vapors while conducting an inspection of the area, he received medical treatment for lung irritation.

Employee was unplugging a drain hose when he was sprayed with materials which got under his safety glasses and in his eye.

Employee inhaled some chemical fumes causing irritation

Employee alleged that he tripped on a some material and fell on his right arm while placing items on a forklift.

Employee alleged that he strained his neck while riding in a fuel truck.

During material movement driver stepped out of semi tractor and a rock rolled under his left foot when he stepped on it causing the driver to jerk to keep his balance which in turn popped his left knee.

While the employee was driving UTV down to the North Access road to lower plateau he made S-turns and the UTV rolled on side.

Employee stated that he tripped on a bolt stud that was sticking out of the concrete, this caused a pop in his ankle, the employee decided to go to the doctor on 7/17/13

Employee was preparing 8' section of 10" pipe to be welded and installed. Employee unbolted upper portion of pipe to install purge valve. upper section of pipe came free began to slide off rack, employee grabbed for pipe and his left thumb for caught between pipe and angle iron of pipe rack.

The employee was completing a blow down process on a press when he jumped over some process lines and sprained his knee

Employee was checking his grinder and putting the handle to the other side and pushed the trigger and it started and came in contact with his left index.

Employee was using a Metabo Grinder. The Metabo bounced back after coming in contact with the 1 1/2" pipe being cut. Metabo blade contacted employees middle left finger.

Employee reported that he had hurt his left shoulder, he alleged that it might have occurred when he was putting together office furniture and disposing of tires into a dumpster

Employee was securing safety lanyard to life line with left hand when left shoulder popped dislocating shoulder.

Fracture - Left Thumb - Struck with 8 lb beater

While moving a 6" pipe in pipe rack #4, employee had his right index finger caught between the pipe and a steel beam.

Employee was trying to close a 6 inch water line valve when he felt a strain or pull in his right shoulder/bicep muscle. At the time he didn't feel the need to be seen by a physician, he later requested that he be seen for observation.

Parking lot was iced over from the previous nights snow storm and employees were waiting to get the ok to go to work. GF went to truck to get a sign in sheet to take names of those who were there. Slipped on ice and dislocated left knee.

Employee was loading hard insulation shields into a man-lift for installation when he pinched the middle finger on his right hand. The pinch resulted in a laceration that required sutures to close. The employee was released back to work without restrictions.

EE was pulling a 3 conductor number 6 cable with eight other miners, the pull was 450 feet. Men were spaced about 50 feet apart in cable tray. The pull was on 12/8/2012 and the injury was reported on 12/10/12. The injury was to the left shoulder.
