EE parked his drill very close to the highwall, we assume to mark the drill holes. When getting back into the drill, he slipped and fell approx. 75' off the highwall. He broke his femur and has massive injuries to his left eye. His condition is still being evaluated.

Propped ladder up on 5110 track to climb on to service at 5:00 am. Ladder slid in mud lost footing hitting right chin and knee on track causing knot and swelling approx. 1 hr later contributing factor, mud, oil, grease, and water.
EE WAS CLIMBING UP A LADDER TO GET ON THE GREASE TRUCK, LADDER GAVE WAY AT THE BOTTOM CAUSING HIM TO FALL TO GROUND LANDING ON HIS LEFT LEG.

EE was moving the 6" water pump at pond 3, he was talking the discharge hose loose, it stuck he pulled hard on it. causing it to come loose at once causing him to fall backwards with it, he hurt his left hand, left leg and ankle. said it was just sore and did not want to see a dr. he told the mine manager about it the next evening at approx. 4:30 pm.
EE WAS STANDING ON TOP OF A 6'STEP LADDER WHILE WORKING ON THE ROCK TRUCK WHEN THE STEP LADDER SLID OUT FROM UNDERNEATH HIM CAUSING HIM TO FALL DOWN THE LADDER. WEATHER CONDITIONS WHERE COLD & ICY, AFTER HAVING APPROX. 5"OF SNOW THE DAY BEFORE. NOTE: EE WORKED THE NEXT DAY.
OPERATOR SAID THAT IT WAS A FREAK ACCIDENT IN THAT HIS FEET GOT TANGLED UP IN THE STEP. (RAINSTORM MAY HAVE CONTRIBUTED TO A HURRIED EXIT FROM MACHINE.) STEPS WERE INTACT AS IN NEW FACTORY CON DITION. HUMAN ERROR APPEARS TO BE THE CAUSE OF THIS INJURY IN THAT AS LATE AS 7/22/02 SLIPS AND FALLS/3 POINTS OF CONTACT WAS OUT MONDAY MORNING SAFETY MEETING FOCUS.
SLIPPED WHILE STRADDING A HANDRAIL ONT HE NO2 DML INERSOL DRILL HE WAS TAKING A SHORTCUT FROM ESTABLISHED WALKWAYS. THE EE AALLEDGED THAT HE SUSTAINED GROIN OR TESTICLE INJURY.
CLIMBING ON A 988G LOADER, BOOTS WERE MUDDY CAUSING RIGHT FOOT TO SLIP AND TWISTED KNEE.
