While performing routine maintenance, the wrench the employee was using slipped off the bolt head, causing the employee to lose his balance. The employee cut his hand on a sharp piece of coal when he used his hands to brake his fall. Employee recieved five stiches at the local emergency room.

After fueling the 16g grader, the ee was dismounting the 16g with associated fuel hose. Due to stiffness of the hose, it twisted causing the ee to fall. EE landed on his right hand jamming his elbow.

EE WAS CLIMBING UP THE LADDER ON A EUCLID R90 TRUCK WHEN HE LOST HIS GRIP CAUSING HIM TO FALL ON THE GROUND ON HIS BACK.

EE GOT OUT OF THE CAB TO TALK TO THE FOREMAN AND WHEN HE WENT TO GET BACK IN, HE SLIPPED ON THE MUDDY TRACKS CAUSING HIM TO FALL & CATCH HIS LEG BETWEEN THE SIDE ARM OF THE BLADE & THE TRACK.
