It was beginning to get dark and the employee, as part of his duties, was preparing to power up one of the portable light towers for night work. He was walking around the tower and said he slipped as it was muddy from a recent rain and fell against an out rigger leg at the back of the tower injuring one of his left ribs.

Showing all 3Employee had been running the 988H Loader and had parked the unit at the end of his shift. Just prior to the incident he had exited the cab carrying his lunch box and a thermos. As he was coming down the first set of stairs he missed a step and fell hitting his left hip on the landing below the stairs.
Employee was clearing material from under a primary crusher. When he stepped backward he steeped on a rock that caused him to lose his balance. He fell with an outstretched right arm. He sprained his right shoulder (right rotator cuff) as a result of the fall.
