Employee was hand-salting an icy area on the haul road. EE was spreading calcium chloride when EE slipped on the ice and fell catching self with EE's left hand. After an x-ray at the local emergency room it was determined to be a fractured left wrist. Also, the next morning EE's left knee swelled. Received another x-ray, no fractures.

Employee was walking to water treatment pond after it rained when he slipped and fell in the mud, twisting his ankle. This caused his left ankle to be dislocated and left leg to break.
Employee slipped and twisted back while going over berm to start and fuel pump. Heavy rains overnight.
On 1/25/06 it was reported to us by Cougar Trucking that one of their drivers had fallen off of his truck at our Ernest site on 1/16/06. The employee was fixing his tarp outside of his truck & slipped and fell to the ground (bruised groin/tailbone). He continued to work the rest of the day & did not report this incident to anyone at the site.
