Employee was on conveyor, 12'-14' high, lubrication rollers. Plant operator could not see the employee, blew the alarm (for all personnel to get clear of plant) and started the plant. Employee said employee did not hear alarm, fell, fracturing employee's left wrist and sustaining a displaced fracture to the right arm.

Showing all 4Rocks lodged in loader belt. Employee was instructed to stay with loader while proper safety procedure was put into motion by other employees. Employee willfully climbed on loader bucket and attempted to remove rock, subsequently falling and striking face on belt cover. He broke a tooth and required stitches to his lip.
Employee was removing rock from sand screen. He lost his footing, slipped and fell on rocks next to him. Sustained bruising and soft tissue contusion. Awareness of surroundings, conscious of footing and balance would help to not repeat accident.
EE was walking up conveyor belt to clear any large boulders that might be stuck on shaker chute when he tripped and lost his balance from his own two feet. He hit the shaker on his left side of his chest. He got up and told his supervisor.
