EE states he stepped off a skid steer onto uneven ground, twisting his right ankle and causing muscle strain. EE was placed on modified duty 12/9/10 and released to full duty on 12/16/10. There is no lost time associated with this injury.
EE states a co-worker was pulling a wheel barrow up the stairs when the wheel barrow slipped out of the co-workers hand the EE fell on his knee getting out of the way. Medical treatment was given and the EE missed one day and RTW to full duty on 5/31/10.
Employee stepped off Cogar feeder onto loose material and twisted right knee. Employee suffered muscle strain and was placed on restricted duty.

He was terminated for other reasoning, left the site & came back hours later and told the supervisor he had an accident before being terminated. EE was walking from one area of the site to another, slipped on concrete form pins and fell on his right shouldr, fracturing it.
