EE STATES THAT HE WAS PULLING A HOSE AND SLIPPED WHILE WALKING ON A LIME PAD. STATES HE JARRED RIGHT KNEE AND CAUGHT HIMSELF WITH RIGHT LEG SLIPPING UNDER HIM. EE DID NOT FALL TO GROUND. EE'S TREATING PHYSICIAN TOOK HIM OFF WORK ON 08/20/2007.

EE SLIPPED ON RAILING AT 51 CAR LOADING FALLING TO KNEES ONTO WALKWAY GRATING. EE SUSTAINED A PUNCTURE INJURY ON HIS RIGHT LEG JUST BELOW HIS RIGHT KNEE. WOUND REQUIRED SUTURES TO REPAIR. ALSO BI-LATERAL SHOULDER PAIN. EE ON RESTRICTED DUTY.

EE was walking when he stepped on a hose causing him to fall backwards. EE got up experiencing pain in his left knee. EE placed on restricted duty.

EE was working on a pump when he struck his head on an overhead pipe. He lost his balance and fell, striking his left leg against a flange and his right elbow against a pipe. On 07/23/07, EE was placed on severe restricted duty by his treating physician, no use of right arm. Not able to perform his regular duties, EE was placed out of the plant. First LW day will be 07/24/07.

EE was walking to the time clock prior to the start of his shift when he tripped and fell against a support beam. He struck his right arm against the beam and landed on his right hand. On 05/04/07, an MRI revealed a small fractured bone on his right hand. EE placed on restricted duty on 05/05/07.

EE's right foot slipped off of a walkway and foot slipped into hot condensate causing a second degree burn on his foot. Restricted duty.

EE tripped on rubber mat and fell on his face. EE felt pain in right shoulder. Also abrasions on knees and hands. On 11/6/07, EE had surgery on right shoulder to repair torn rotator cuff. EE losing time.

EE tripped on grating and experienced pain in his right hip. On 2/20/2007, Plant doctor gave EE a prescription of Utram for pain.

EE was ascending a ladder when he struck his left knee on a ladder rung. On 04/24/07, EE was placed on restricted duty and was not able to work. He started losing workdays on 04/24/07.

EE was walking on the facility dock when he slipped and fell. He broke his fall by reaching out with his left arm. EE felt a pain in his left shoulder. On 04/27/2007, EE had surgery on his left shoulder. EE is currently losing time.

EE was walking in his assigned area when he stepped on an uneven surface, rolling his right ankle. EE was diagnosed with a fractured right ankle.

EE was standing on the access ladder of a payloader. He lost his footing and landed on the payloader fender. His left hip sustained a laceration about two inches in length that required sutures to repair. No lost work days. No restrictions.

EE was descending a stairway when he slipped. He caught himself on the stairway handrail and felt a pain in his back. He was sent to a private physician who took EE off work. EE currently losing work days.

EE was walking on an elevated walkway at the bottom of a manlift. He stepped into the belt return opening and lost his balance. He landed on his right elbow. He sustained a laceration on his elbow that required sutures to repair. Minor scratches and bruises on his upper body. No restrictions or lost work days.

EE slipped while ascending onto a scaffold. He reached out to some adjacent piping for support. He felt a pain in his right shoulder. On 05/02/2006, EE was referred to a Treating Physician. Diagnostic testing indicates that EE will require surgery to repair a rotator cuff tear. EE placed on restrictive duty on 05/02/2006.

EE was ascending a ladder. He stepped off the ladder rung and onto the landing. He missed the landing and landed on his right knee. On 04/04/2006, EE had surgery on his left knee to repair a torn meniscus. EE is losing work days.

EE was ascending a ladder when his foot slipped. He caught himself by hanging his right arm over one of the rungs. He felt a pain under his right arm. Plant doctor placed EE on restricted duty on 02/14/2006

EE was pushing on some steel rods when he slipped and struck his chin against a rod mill discharge chute. He sustained a quarter inch laceration which was closed by the plant doctor using super glue.

EE was walking on the into a building while it was raining. He slipped on a wet spot and fell. He broke his fall by extending both arms out. He felt a pain in his right shoulder. Diagnosis was a torn rotator cuff. EE had surgery on 12/26/2005 to repair the torn rotator cuff. EE started missing days on 12/27/2005.

EE had allowed mud to accumulate on his work boots. He was walking on a concrete pad and slipped as result of the mud accumulation. He felt a pain in his left groin area. His personal physician diagnosed him with an inguinal hernia. Surgery to repair will be done on November 10, 2005.

EE was using a valve handle to open a six inch plug valve. The valve handle broke and the EE fell backward. He landed on his backside. He experienced pain in his lower back. He was taken to a local hospital for evaluation and the ER doctor took him off work for two days. EE is on restricted work duty.

EE was working inside of an evaporator vessel. He slipped while standing on an internal ledge and fell approximately three feet onto the evaporator floor. EE sustained a severe contusion to his right shoulder. Treating ER doctor took EE off of work for one day. EE reassigned while on work restrictions.

EE was walking down an inclined walkway. He stepped on a slippery spot, lost his balance, fell backward and landed on his right shoulder. Plant physician placed EE on restricted duty and prescribed PT 3x/week.

EE did not report this injury until 08/17/2005. EE sustained a right knee sprain when he stepped on a rebar that was sticking out of the ground. He felt a pop in his right knee. Plant physician prescribed pain medication to alleviate the pain. EE was placed on restricted duty.

EE stepped onto an uneven walking surface and felt a "pop" in his right knee. Taken off of work by the plant physician on 07/28/05.

EE was walking down an inclined walkway. He slipped and reached out to the handrail. He felt a pull and a burning sensation in his right elbow. On 08/12/05, the plant doctor injected his right wrist with cortisone. EE placed on work restrictions.

EE stepped over some process piping and felt a pain in his right leg. On 06/24/05 EE begin losing work.

EE was walking on a slippery (wet) driveway. He slipped and twisted his left ankle. On 4/22/05, EE had surgery to repair hairline fracture.

EE was descending stairway, when he reached the bottom and stepped onto hard surface, he twisted his right knee. Placed on restricted duty.

Employee was attempting to get off a barge. His footing slipped and he scraped his left shin on the access ladder. The abrasion required sutures to repair. He was placed on restricted duty on 04/05/2005.

EE was working inside of a scalloped floor tank. The floor was slippery and the EE started to slip. He locked both knees to keep from falling and he felt a pain in his left knee. On 03/31/2005, EE was placed of restricted duty and reassigned to a light duty position.

Employee was walking on top of some alumina airslides when he struck his head on an overhanging piece of pipe. He went backwards and fell 45 inches to the ground. Employee struck his head, right shoulder and right leg on the airslide. On 03/09/2005, employee was taken off work by his treating physician.

On 03/01/05, employee fell against some structural support beams and striking his right rib cage. On 03/07/05, employee was placed on work restrictions. No ladder climbing, no lifting > 10# with right arm and no use of sledgehammer.

Employee was stepping over a pipe when EE twisted right ankle. On 02/08/05, plant doctor applied air splint to immobilize the ankle. Working with restrictions.

Employee was descending a stairway. When he stepped off the last step, he slipped on some mud and twisted his left knee. His personal physician took the employee off of work on 01/22/05.

Employee was ascending onto the top of a barge. He slipped and pinched his right hand ring finger between ladder and wall. On 09/02/2005, plant physician administered a steroid injection to the affected finger. On 1/23/2006, EE had surgery on right hand. Surgeon found cyst on the bone; removed cyst. Returned to work on 1/27/2006 with restrictions.

A Building Trades Mechanic slipped and fell while attempting to step over a four inch portable pump suction hose. The employee's fell toward the portable pump and struck his head against the pump. Employee was wearing a hard hat. His head glanced off of the pump and he struck his left ear against the pump. This resulted in a laceration that required sutures to repair.

Employee was walking down a stairway. He misjudged the distance to the last step and overstepped directly onto the concrete floor. X-ray reveals a hair-line fracture of an ankle bone.

A Mobile Equipment Operator had been working inside of a ship hold collecting bauxite with a pay loader. As he was ascending the ship hold ladder, to exit the hold, a foot slipped and he lost his hand grip on the ladder rung. He fell approximately five feet, landing on his right side. He climbed out of the ship hold and reported the incident. Hairline right rib fracture.

EE was moving a section of grating in place on a platform. He decided to do it on his own and not wait for help. EE was tied off to a handrail with one lanyard & was using the other to drag the grating. He pulled the grating too far & it fell 15 ft, causing him to be pulled down against the existing grating he was standing on. EE was released to return to work the following day.

EE was walking to his office at the beginning of his shift. He stepped on some slippery material on the roadway and landed on his right knee. Eventually had surgery on his right knee on 09/16/2004.

EE was on clean up duty. He was using water to hose down the deck of a bauxite unloader. He slipped on the wet floor & reached out to an adjacent handrail to break his fall. He felt a pain in his right shoulder & wrist. Plant doctor diagnosed as a mild shoulder and wrist sprain. Plant doctor prescribed prescription strength medication for pain as needed.
