The employee was cutting timber for a raise crew. As he lifted a scrap piece of square timber 12"x12"x5' long he got off balance and when he caught himself he felt pain in the lower abdomen mid-line down to the groin.

EE was cleaning threads on a cone crusher while other crew members were extracting the mantle. A worker yelled at EE to free the chain on the chain hoist and he stepped into a 1' diameter circular hole that had not been covered after the extraction. This injury was reported as a minor injury and because of persistent symptoms we referred him to a Dr. on 8/17/12.

The employee had placed a jack leg drill on the muck pile which was on an inclined plane. He then stepped back to retrieve some rock bolts and when he turned back around he slipped and his chin struck the crows foot on the leg which because of the slope was a little over waste high.

The employee was installing a header and tried to gain elevation by standing on a wood block appx. 10 x 10 x 18 inches. The Block rolled which caused him to fall. He put his arms out to break his fall which caused a hairline fracture to the radial head when his arm contacted the concrete floor.

EE had stepped into a two foot trench to help a fellow employee lift a rammer over some underground utilities and when he stepped out of the trench, his knee gave out on him, causing him to fall back into the trench. His knee is the only injury that he received.

Employee was knocking lacing to get back into stope after shooting a brow round. He had knocked 3 lagging and placed them over his joker chute. He tripped and fell in the joker chute. He broke his right leg and had lacerations on his head.

After reviewing the data from the ESG seismic System, there was a seismic event that occurred 80 feet below where the accident occurred there was noise-popping and snapping in the face of the heading EE was working at which startled her, she turned in response to the popping and tripped and fell with hard hat coming down across the bridge of her nose.

Employee was barring in a hung muck chute to get the muck to run. A large boulder came down and caught the bar causing it to strike the employee knocking him down, hitting his head on the muck car. This caused two lacerations requiring sutures.

Employee was standing on the third rung of a ladder. Employee was attempting to lift and move a pan feeder segment. When he stepped down his foot went between the second and third rung causing him to fall straining left knee.

9/26/06, EE filled out a minor inj report on his left knee after he stepped out of the truck he was driving to lock a gate and the roadway gave way into an old culvert. EE has con't to work without missed shifts or restrictions. 0n 1/30/07 EE had arthroscopic surgery on his knee. 1/31/07 EE ret'd under restricted duty.
