Showing all 5Employee was helping change the paddles on a scrubber. Employee was standing on the frame or the scrubber tower guiding the scrubber's into the chamber when employee lost balance and fell on to catwalk about 5 feet below. Employee was wearing fall protection but was not tied high enough to dampen the fall.
The worker heard air leaking out of a bladder used for the final cleaning of the slurry line. When he went in front of the bladder it let loose and struck him in the stomach knocking him backwards causing him to lose balance and hit the back of his head on the pump behind him causing a small cut on the back of his head. That night he received 2 staples to keep cut closed.
Employee and 3 others were moving sidewall panel. While walking backwards, he stepped in a small trench. When he stepped down he jerked upwards and felt a pull in his scrotum. Went to the doctor 10/20/2012 who confirmed Scrotal Varicocele.
Employee exiting "change house" missed the bottom stair step, his ankle rolled under him and he broke his right fibula.
