EE had wet down the muck-pile, while rolling up water hose EE slipped on some residual hard-cap paste landing on EE's right arm. At the end of shift EE requested to be seen by the Pogo physician's assistant where it was determined to send EE to Fairbanks for an x-ray; which was definitive for a fracture. EE is onsite currently w/restrictions and a splint/cast.

Showing all 12The employee was working at the face of the 857 East loading a round. When EE turned to retrieve supplies, EE's foot was caught in some mud and EE's knee "twisted" and popped causing the employee fell to the ground. EE was taken to the local ER, treated for an apparent patellar dislocation, given a splint, released w/out restrictions and is to follow up with personal physician.
Miner was stepping down off of Kubota 5 in the 1908 stope access. Miner dismounted in the front facing position, left leg first using 3 points of contact. After reaching the ground with left leg, miner fell to the side losing balance, feeling immediate pain and heard "popping" sound.
Person was working on powder boxes in the Construction Shop when he fell hitting his head on some machinery. Cutting him above the right eye and the right upper lip
roofing crew started work & moved onto the mill roof to access conditions (ice wet frost) ee moved from manlift after testing his footing he walked approx 60 foot away from manlift test wet areas on roof as went. Turned to move up the roof line to remove a tarp & when he pivoted on what he thought was a wet area (black ice) he lost his footing and fell striking his shoulder.
Employee loaded a pallet with scrap metal and was attempting to push the pallet when his hand slipped, causing his front tooth to hit a piece of angle iron. Resulted in a crack in his front tooth.
Worker was dismounting jumbo drill and rolled his ankle resulting in a sprain.
Employee was exiting outside double doors located in the paste plant. While stepping over a raised berm at the doorway his left foot slipped on the concrete floor, as a result employee fell to the ground. Employee was wearing a set of "Yak Trac" traction devices on his boots.
While reaching for a split bolt to load onto bolter carosel, employee felt a pop sensation in left knee area. The knee continued to pop throughout the day. Over night the knee became swollen and very sore.
Employee slipped on loose gravel. He was not carrying anything, slip resistant footwear was being worn. sprain to right knee. Employee is relased to modified duty on 9/25/2010.
While climbing down out of Grove 35 ton crane using the ladder steps, when employee got to the second to last rail his foot slipped resulting in his hands slipping down the rail. Employee's right finger became twisted in the railing resulting in a fracture of the right ring finger.
Employee was helping to unplug a line in the mill when he was startled by a noise and some slurry coming out of the breather pipe, as he moved away, employee slipped and fell onto a metal valve puncturing the skin on his lower left leg.
