Accident was reported on Wednesday July 31st. 5200-186 - pulling chutes, jumped off motor to pull chute twisting right knee when heel of boot caught on motor hitch.

Employee slipped and fell on ice/snow straining upper back, arm, and both legs. Went back on January 21st, Local clinic sent employee home for 3 days to rest and recover. *EE was diagnosed with fracture to left ankle on February 22nd; taken off work as of February 25th. Surgery scheduled after appointment on March 26th.
5200 Upper Pocket, spot cage at pocket landing step out, slipped on cap of landing.

3200-019 get personnel gear off locomotive, slipped on mud landing on rail, catching rail between fingers of right hand. Went to ER, negative x-ray, was released but Dr wanted PT to be seen at clinic, waited two days for appointment - at that time released to full duty.
Miner in 3400-048 Raise, tripped and hit knee against footwall, small cut. Infection, miner off work for treatment of infection.
Walk from dry to mechanic shop, slipped on ice, grabbed rail, pulled right shoulder. Employee went in for MRI on February 16th. Employee's physician took employee off work as of February 21, 2017 pending physical therapy and possible surgery.

EE climbing up step ladder; left foot got caught up between rung and hinge on ladder - EE lost balance and fell to the floor landing on right shoulder and compressed left knee.
Miner was hanging water line, lost his footing. Trying to catch himself, he grabbed a monster mat and cut his left pinky finger.
Employee went outside mill to take a break - sat on makeshift bench fell back jamming left shoulder trying to catch himself. Went to local clinic after shoulder continued to bother him on October 9, 2014
An employee lifted a piece of 40-lb rail with another miner. As they were walking the rail to where it was going to be installed the employee tripped on a 1" hose that was laying in the walkway. The employee fell and the rail landed on his right shin causing a severe hematoma.
Employee was cleaning the ground floor of the rock house with a 1" water hose. He was walking forward near the base of the E-45 conveyer foundation while pulling the 1" hose. The employee's left foot caught the foundation and he tripped and fell on to his left side and on to the concrete floor. He was walking east towards the rock house room.
EE was examining the cleaning cells at the mill which sit on a slightly elevated floor. The floor is constructed of metal grates and there is a 6" step up to the elevated floor. When she left the area she misjudged the step as she was leaving the elevated floor. This caused her to fall on her knees resulting in a L. ankle sprain, and a sprain and a laceration to R. knee.
He had just finished moving the electricians utility truck from it's covered parking area so that he could have access to move a fan out of the shop with the forklift. He parked the pickup in an uncovered area. When he stepped out of the vehicle his foot slipped on the icy surface. In an attempt to catch himself he slammed his other foot into the ground hard enough to cause injury.
Employee was bolting and dropped his jackleg. when he went to pick the machine up he slipped and fell and cut his wrist on a rock.
The employee was rebolting a track drift as part of a 2 man crew. There was a hose stretched down the center of the rail which was supplying air to his partners machine. He started to step over the hose and at the same time his partner pulled some slack causing him to trip. Since his toes were on the ball of the rail when he fell the forward weight transfer caused fractures.
Employee had just finished drilling a round with a jackleg drill. His partner pulled the steel out of the chuck and as he stood the machine up he lost his footing and fell backward pinching his hand between the machine and the corner of a rib mat. This caused a laceration to the web between his fore-finger and thumb which required sutures.
