Employee finished changing the brake calipers on the hose reel of the highwall miner. EE was retrieving a fitting that was laying on the hose reel frame when EE slipped and fell. EE tried to catch self with left hand and EE's fingers went through the metal grating injuring the hand and fingers. EE also struck EE's head as EE fell resulting in a small cut above the right eye.

Miner was walking in outby entry and rolled ankle causing a sprain.
Employee stated was walking around scoop when EE tripped on a box and fell across the bucket of a scoop striking EE's ribs. EE finished the shift and sought medical treatment and was given a full release but was diagnosed with a rib fracture. EE returned to work on EE's next scheduled shift and has not missed any work.

Employee was in the process of hooking up a catheads on a Highwall Miner car. As the car started, ee bent over to pick up a control cable and place it on the car. As ee stood up and began to walk ee stumbled and fell. This resulted in receiving a laceration to the right forehead. It is unknown as to what ee struck head on.
Employee was mounting an 850 John Deere dozer, lost footing due to muddy conditions and fell against dozer tracks striking rib cage.

EE stated that it was wet and muddy in this Entry. Employee was transported to RGH by ambulance.
While unloading roof control supplies in a scoop bucket, injured slipped and fell twisting left knee and bumping head. While at hospital, employee had a severe allergic reaction to the contrast dye injected to perform a CT scan and was hospitalized.
It was raining and the employee had an umbrella in hand. Employee was walking around the corner of the security post in order to enter the building. Employee caught the toe of employee's boot and fell forward landing on left arm on the pavement. Employee fractured the radius bone in left arm. Employee's first day away from work was 2/20/2018.
Employee was in the process of installing new skirt boards in the R-4 tailpiece, after unhooking the chain that was used to place the new skirt boards on the belt, employee was walking on the belt back toward the tailpiece and slipped and fell backwards. They attempted to catch themselves, causing a small "chip" fracture in their wrist.
While stepping down from the mounting ladder on a drill to the ground, the employee started to turn and walk away from the drill. EE's right foot stayed stationary and the rest of body turned in the direction EE was turning causing pain to right knee and right hip. Employee did not fall. Employee's first day away from work was 1/24/18.
Mechanic was disembarking off of the fuel truck. Misjudged the step and fell backward, hitting their head on the open door, edge of the service truck.
