Contract employee was leaving the house of the dragline via the staircase. Once EE reached the bottom step, EE lost EE's balance and began to fall to the dragline shoe below (25 inches). EE's momentum carried EE forward, where EE fell off the dragline shoe to the ground below (51 inches). The impact to the ground resulted in 2 fractured vertebrae and lacerations to EE's face.

On 1-19-26, a Lube truck operator stumbled on dozer tracks and fell to the ground sustaining injury to their knee. Operator was able to work up until 2-7-26, when MRI results were received making this a lost time injury.

Employee fell on a seat arm rest on the shift change bus injuring their left rib area. Doctor diagnosed as first aid injury with no restrictions on 11-11-24. On 11-21-24 employee contacted safety indicating was still having discomfort. Employee seen by medical professional on 11-22-24 and placed on no work beginning on 11-23-24 until follow-up appointment on 12-3-24.

Employee was stepping over #2 hoist coupling to inspect #1 hoist motor and their foot slipped causing them to lose their balance. The employee reached out to catch themselves from falling hitting their left hand causing a dislocation of their left ring finger.

Employee was walking around the back side of a wheel pump and lost footing due to a washout. As the employee stumbled, their left foot went downward into the washout. As their right foot came down to keep them from falling, the right side of their knee came in contact with a piece of angle iron on the wheel pump causing laceration on right knee.

Mobile equipment operator of a 785D hauler fell through the access ladder approximately 4 1/2 to 5 feet from the platform to the ground while reaching to pick up their backpack and cooler off of the platform.

Employee had just finished parking and chocking the water truck in the equipment parking area and while walking toward the shop to get on another piece of equipment, the employee rolled their left ankle. Employee stated that the lighting was good in the area and that EE's boots were not the greatest.

At the end of shift on 4-13-22, at approximately 4:40 am, a dozer operator that was working at the A2 area ramp was in the process of dismounting their dozer. While dismounting the dozer, the operator's foot slipped off the last step causing them to fall to the ground causing a strain / sprain to their left knee.

Employee was operating a farm tractor when the engine began to overheat, the employee lowered the loader and shut the tractor down. While opening the hood, the employee stepped over the loader tilt linkage when employee's foot slipped causing left knee to strike the loader tilt linkage resulting in a laceration about 3 inches below the knee requiring stitches to close the wound.

Employee stepped out of car in the employee parking lot prior to work shift and slipped on ice causing them to fall to the pavement. Employee reported soreness with hip, shoulder and bicep but did not request medical attention. Later on 4.8.21, employee started a series of physical therapy treatments which made this incident reportable to MSHA.

Employee was in the process of removing abandoned discharge line that was stuck in the mud, when EE lost balance and fell backwards (same level fall) striking the left side of face on the road causing dirt, sand to enter left eye. Employee taken to local clinic where eye was flushed and RX given for eye ointment for the irritation.

Employee was on the office porch. As ee walked to the door, ee fell injuring left ankle. The injury was evaluated by a doctor and initially diagnosed as a strain sprain. An MRI was performed to confirm and showed a fracture to the talus bone. The results from the MRI were reviewed with doctor on 12-13-2018 and resulted in classifying this injury as an MSHA Recordable.

Employee was dismounting an elevated platform using steps and handrails with 3 points of contact. One foot was on the ground, and when the second foot was going from the last step to the ground, the original foot slip out from under ee and caused them to fall.

Employee was pulling a 6" poly pipe line (for pumping water) with a nylon strap. The strap slipped (slack) and allowed employee to stumble backwards. Employee felt pain in lower back as a result. The pain got better but then worsened. On 11-8-17 employee was placed on restrictions which Luminant cannot accommodate. 11-9-17 was first lost time day.

Employee was coming down a set of steps just outside. As EE took last step with EE's right foot to the ground, EE heard and felt a pop just above the knee. EE fell to the right side on the ground. An ambulance was notified and employee was taken to Longview Regional Medical Center where the mechanic was diagnosed of having EE's right quadricep tendon ruptured.

A dozer operator was discovered lying beside a portable toilet in the field. EMS was contacted and the employee was transported to a local trauma center. It is believed that the employee passed out after exiting the toilet and fell back hitting head and back against the toilet's skid. Employee was diagnosed with two fractured vertebrae in lower back.

Electrician was pulling trail cable to put back on the cable shoe and slipped in the mud and fell face forward landing on hands and knees. The electrician felt pain in lower back. The electrician continued to work to get job done. The electrician decided to report the injury on 03/16/2017 as the pain worsened and was very sore. Rain and mud were a contributing factor.

The Dozer Operator was ascending back to the cab, lost footing striking right shin on the push arm of the tractor. The Operator did not know that the laceration had occurred until the Operator was back in the cab. The conditions were wet and muddy due to the rain event that was occurring.

While pulling on oil refill hoses the employee slipped on some oil on the PM bay floor. After slipping employee felt a twinge in knee. Employee was taken to the doctor on 1/30/17 and released full duty. On 3/1/17 employee was not able to return to work because of knee causing this to become recordable. Employee was scheduled to start physical therapy and see an Orthopedic surgeon.
