Showing all 7Employee was leaving an office and walked around a barricaded area and slipped in the mud. EE braced self on the dumpster to keep from falling and right foot turned outward resulting in a fracture.
Supervisor was walking to EE's office when EE slipped and fell backward landing on EE's right upper back. Seen at Gulf Coast Occ Med and returned to work after negative x-ray. Reported change in symptoms on 1/29/2018 and was seen by Ortho Dr. on 1/30/2018 and given restrictions and medications.
Walking in Jamaica building. While walking fell in hole. Injury left knee.
Employee standing on elevated platform pulling 1B filter cloth to straighten out on unit. While pulling on filter cloth lost grip of cloth fell backwards from platform approximately 5' in height to lower level. Landing on back.
Employee walking up stairway to work level to take down scaffold stumbled on sling ee was carrying. Lost balance causing ee to fall forward. As ee was going forward caught self with right arm to stop fall and felt pain in right shoulder. 3-24-17 Evaluated in medical first aid treatment. 4-10-17 F/U evaluation scan shows rotator cuff injury. Restricted duty.
Employee was walking on top of a dry bulk barge and missed a step, causing employee to fall. As employee was falling hit head and back on the dock. The result of this contact with the dock caused the employee to have Transverse Process fractures in lower spine and bruising to head and back.
