Showing all 21The Employee was going to turn off the water valve after filling up the tank on EE's steam truck. The employees foot slipped and EE caught self, felt a sharp pain in lower back. Went to the doctor a few days later x-rays shown nothing, given time off with meds.

Employee was climbing upon a Rock Truck, when EE's foot hit the first platform on the lower level EE's foot slipped and EE lost grip falling on to the ground approximately 3-4 feet on EE's left side. EE was transported to the Hospital and later released that evening.
Employee stopped EE's service truck in an area where a drill had been repaired. When EE walked around to shut a cabinet door EE stepped into a drill hole. EE received a sprain left ankle and a small fracture in EE's foot.

Employee was exiting cab of dozer. When they stepped on the deck it had oil on the deck and they slipped and fell hitting an elbow on the deck and hit their tailbone on the dozer track.
