The employee was stepping out of EE's pickup and twisted EE's left knee.
While doing pre-op grader right rear side, notice the engine compartment door was open, crawled up to close the door, the latch handle was broken, went back to cab to get pliers to close the latch, after closing the latch, crawling back down slipped on the small lower foot step. Reached to grab the hand rail to keep from falling off the grader and somehow cut left hand.
Was at pond 11 checking water pump fuel level. While walking slid off the berm into the mud. Right foot stuck in the mud about 5" deep and I twisted ankle, pain also felt in the lower knee area.
While dismounting a D11 Dozer, the employee's foot slipped on a track grouser, causing a loss of balance and resulting in the employee striking EE's head on an iron grab handle.

An employee was dismounting a dragline. EE mis-stepped as descended one of the last stairs and fell to the ground. EE was later diagnosed with a torn tendon, which will require surgery to repair.
The injured employee was stepping from the lower deck of the excavator to the upper deck. EE missed the step and hit shin on the step. EE received a cut that required stitches to close, making this incident reportable.
The employee was fueling a water pump. The employee slipped in the mud and rolled left ankle causing a sprain. The employee continued to work. On 7/17/23 the employee received medical treatment for the injury, making this incident reportable. The employee was released to full duty.
An employee tripped while mounting a haul truck. EE felt pain in the shoulder as EE caught self. The employee was evaluated at the clinic and later (on 6/3/23) received medical treatment for the injury, making this reportable. The employee was not released to return to work.
EE was in the process of switching out pieces of equipment. As EE was walking from loader over to Dozer, EE slipped on snow and fell backwards.
The employee exited a vehicle and began walking across a parking area. EE slipped on ice and fell on EE's right leg/ankle. EE was taken to the clinic for X-Rays and was diagnosed with a fracture. The employee was not able to return to work.
Employee tripped in mud, landing on elbow. EE requested medical evaluation. EE was not released to work pending further tests. (MRI)
Employee dismounting boarding ladder on last step knee gave out and fell. Employee did not want medical evaluation. On 11/15/2022 employee, requested medical evaluation. Seen at medical facility, employee was released to full duty. On 1/09/2023 went in to medical medical facility to have knee checked. Employee was given a Do Not Return To work notice and had surgery scheduled.
Employee stepped off of step stools last/bottom rung rolling EE's left ankle resulting in a sprained ankle resulting in the inability to wear EE's steel toe boots (PPE) to return to EE's normal duties.
The injured employee was in the locker room getting ready to start the shift. EE grabbed water jug and hard hat and turned to leave the area. EE's left knee made contact with the bench and EE fell over the bench. The employee got an MRI and on 8/23 EE was diagnosed with a hairline fracture and was not able to return to work.
EE reported that they had tripped on suction line on pump. EE said EE caught self with left hand and was injured. EE was taken to medical. Results were inconclusive at ER and EE was referred to an Orthopedic specialist. After Ortho evaluation EE was given work restrictions and is now off work pending further tests. First day of lost time was 6/22/22
EE was working at Dragline during a scheduled down day. EE leaned against a handrail for a better view. The weld holding the handrail failed resulting in EE falling forward about 2'. EE struck head on stairway railing resulting in two staples to close laceration. EE returned to work with no restrictions. Handrail was immediately repaired.
EE was taking a step backwards from EE's hauler. The ground was muddy and EE tripped on uneven muddy ground falling backwards. EE reported a stiffness in back and was taken in for medical evaluation. EE was released to work. EE returned to Dr. the next morning and was advised to take time off due to stiffness in back and did not report for next scheduled shift
While dismounting Cat End Dump, EE turned ankle as they stepped from one stair to the next. EE was evaluated at ER and not released to work next scheduled shift 4/4/2022.
Employee was walking along the LW face. EE stepped down from the shield toes, onto the DA Relay bar. EE's foot slipped and EE fell, striking the side of EE's head against the base lift cylinder, causing a laceration to EE's right ear.
Employee started down a 4' ladder into trough of Dragline. On second step down EE did not maintain three points of contact when one hand slipped from ladder. EE fell from ladder and cracked 4 ribs on contact lifting eye in floor of trough.
Employee slipped while dismounting from a light truck. Leg slipped out from under employee, tried to catch self. Strained left leg & left shoulder.
A contractor was dismantling a building that the contractor purchased at auction. Contractor hired two temp workers to help. One of the temp workers fell approximately 20 ft and was injured.
Employee was emptying drain buckets on the number 4 swinger moving between the tub frame. The employee set buckets down to crawl through when their toe caught on the entryway causing them to trip. The employee caught themselves on the frame at this time the employee felt pain in their lower back into their upper left buttocks.
EE slipped and fell while walking from the shower to EE's locker, striking head on the floor, resulting in a laceration which required stitches.
An employee was placing a 5" pin in the back of a truck. The ee's boot heel caught a piece of ice causing EE to trip, EE threw the pin in the back of the truck as EE was falling, EE caught self on the ground with EE's hands and then the pin rolled off the tailgate of the truck and landed on EE's left hand.
While dismounting from a piece of equipment in the shop, a miner twisted a knee. Miner was evaluated by an off-site medical professional and was released to return to duty. An MRI test was conducted later and identified a torn ligament in the knee and required surgery on 3/24/2020 making it a reportable injury.
Employee reported twisted knee from stepping out of service truck on frozen ground on 1/29/2020. Did not seek medical attention at that time. Knee did not heal and medical evaluation was performed and employee was held out of service on 10/30/2020 due to medical restrictions on knee.
