Ee was stepping out of conex holding a box of uniforms and EE rolled EE's right foot on the lift gate. Seen at clinic, diagnosed with right ankle strain, and released with restrictions that do not preclude EE's full duty as a planner.
Ee walking downhill to EE's equipment when EE's knee dislocated, causing EE to fall to the ground. Seen at Golden Valley, diagnosed with strains to knee and back, and released to modified duty.
Ee reports steam cleaning on top of shovel when EE slipped an fell, landing on EE's right side and shoulder. Seen at ER where EE was diagnosed with rib and back contusions and shoulder strain. Seen at ortho on 4/9, diagnosed with AC joint separation. Referred for surgery.
EE was walking past an unoccupied jammer underground when they slipped and began to fall. EE struck out right hand to catch themselves, contacted a broken bolt sticking out of the rib, lacerating right palm. Seen at clinic, wound closed with sutures, and EE released to restricted duty. Site offering modified duty.
EE reports that they tripped over their own feet exiting the cage, caught self with left arm, straining shoulder. EE seen at health, diagnosed with a shoulder injury and referred to orthopedics which they had already scheduled. Orthopedic continued them off work pending follow-up.
IP descended from an AD30 Haul Truck and stepped on a rock that shifted underneath EE's left foot, causing EE's ankle to roll, and EE fell to the ground.
Ee lost footing descending ladder, fell, caught self with left arm. Originally released to full duty, but on follow-up on 4/30, concern for rotator cuff tear resulted in restricted duty and referral to ortho. Scheduled for ortho on 5/26.
Ee reports walking down stairs at flume D, slipped on last step, rolled EE's ankle. Seen at Golden Valley, diagnosed with a closed fracture distal fibula, released to modified duty which site is offering.
EE was connecting a nip, lost footing, twisted and dislocated their patella. Seen at the ER and released to light duty. EE referred for surgical consult and on 3/11, NGM received confirmation from the surgeon that a tear in the knee was caused by the work-related incident.
EE reports stepping down off haul truck, tripping on rock, and falling to ground. Seen at health, diagnosed with a hamstring injury, released to restrictions. On follow-up before next scheduled shift, EE released to full duty.
Ee walking towards connex, tripped on a raised curb. On 2/23 EE requested to be seen by a doctor as EE was still having pain in EE's shoulder. Seen at Golden Health, diagnosed with a sprain, and released to restricted duty.
EE was walking across the parking lot to the autoclave when they slipped on snow covered ice, falling and landing on right shoulder. Their shoulder continued to hurt, and on 2/25 they were diagnosed with a soft-tissue tear, making this reportable for treatment. The EE resigned and left NGM, their last day was 2/26.
Ee was staking voids. EE was walking on uneven ground and EE's foot caught between two rocks, EE lost EE's balance, fell, and felt a pop in EE's left foot. Seen at clinic, diagnosed with a fracture to left ankle, referred to orthopedics, released to restricted duty.
Bolter operator and mechanic were aligning carousel on bolter. Ee using a short scaling bar to rotate carousel for alignment. While pulling back, the bar slipped off the carousel collar, causing ee to stumble backwards and fall, hitting EE's head on bolt rack on front of bolter. Seen at ER, lac closed with staples, released with restrictions.
Ee was walking to clear plugged chute. EE reports rolling EE's ankle on an unseen object. EE reports hearing a loud pop in ankle. Seen at ER, xrays negative for fracture, given ace wrap and crutches, released to restrictions. Site cannot offer temporary modified duty. EE released w/restrictions, site is offering modified duty. EE has no missed time.
Ee tripped on a backpack someone had left in the walkway. EE fell and hurt EE's right shoulder. Seen at ER, released to full duty. On follow-up EE was released to restrictions, and referred for PT if EE does get progressively better.
EE reports left knee turning wrong when getting out of work truck, and ee heard a pop. On C4, EE reports stepping on a rock. Seen at clinic, released with restrictions. Diagnosed with strain, and additional diagnosis pending MRI.
Ee stepped up on cribbing to look at site glass on hydraulic oil tank. EE was holding on to rear access handle on lube truck, the cribbing block rolled from under EE's feet, and EE fell. EE's shoulder dislocated midfall, and EE fell to ground. Seen at ER, referred for ortho follow-up, released with restrictions.
EE was taking wire to a heading. Reports they were walking downhill, stepped wrong, straining ankle. Seen at ER, given brace and crutches, and released to return to work restricted.
EE was getting numbers off a piece of equipment when coworker approached them from behind. Saw them and was startled, causing them to jump, landing awkwardly on the grating, and rolling right ankle. Seen at medical, diagnosed with a sprained right ankle, and released to restricted duty which employee is working.
EE was dismounting dozer, fell backwards off track, landed on bed pad with left hand, causing puncture wound. Seen at Health, diagnosed with puncture, released with restrictions and site is offering modified duty.
EE was unloading truck. As they were walking, a foot caught on grating, twisting their ankle. Seen at clinic, diagnosed with ankle sprain, x-rays positive for fracture, referred to orthopedics. Released to sedentary work, site is providing modified duty.
EE reports tripping while they were going into the locker room to get equipment for the day. EE reports pain in left elbow and shoulder. Seen at clinic and referred for orthopedic consult. At orthopedic consult on 4/3/25, diagnosis of acute proximal radius fracture confirmed and employee put off work pending MRI.
Ee stepped off ladder w/rt leg, reports ee misjudged the last step and ee's lt foot caught, hit the ground, and ee heard a pop. Reports pain when moving at 8/10. Seen at clinic, diagnosed with sprain, and released to modified duty.
Ee was dismounting scissor deck underground. Ee slipped and fell, landing on left wrist. Ee was seen at the ER, diagnosed with a left distal radius fracture, given a brace and released to modified duty pending ortho consult. At ortho consult, ee put in cast and put off work for 3 shifts, then restricted.
Ee was climbing ladder on grader when ee slipped, falling about 3 feet to the ground, landing on buttocks. Seen at ER, diagnosed with contusion to back/pelvis, released with restrictions.
Ee walking to place mill pan at mill press and EE tripped on rubber mat, falling to left knee on grating. Reported incident and practiced self-care. On 3/31 EE sought treatment and filed a workers' comp claim. Released to full duty and referred to physical therapy.
EE was dismounting a piece of equipment. They missed the first step and did not have a good grip on the handrail. They fell to the ground, onto left wrist. Diagnosed with a fractured scaphoid, put in brace, released to modified duty site can offer.
The employee was walking upstairs carrying materials when EE slipped on the top step and fell forward landing on left arm. The contact resulted in a fracture to the arm just above the wrist.
EE reports getting out of truck, slipping on ice, and feeling pain in knee. On 12/2/24 EE saw a provider at the clinic who diagnosed EE with pain in the knee, subscribed physical therapy, and released EE to restricted duty.
Ee reports trying to get out of haul truck and somehow ee's foot got stuck and ee twisted knee. Seen at a health clinic, diagnosed with a sprain, and on follow-up was released with restrictions. Currently working modified duty.
Employee was conducting welding operations on work platform. The hand rail post broke and the employee fell to the lower level. Miner cut knee requiring sutures to close the wound.
Ee reports stepping off bus at queue area to go to ee's haul truck. Ee reports rolling ankle and falling to ground. Seen at clinic, diagnosed with ankle strain. Released with restrictions site can accommodate.
Ee was walking down stairs into pit at the shop. EE slipped on last step, fell back, and caught self with right hand. Felt a pop. Seen at clinic, x-ray positive for fracture. Ee put on restricted duty site can accommodate.
EE was bolting, and a bolt fell out of EE's fingers. The employee powered down to reinstall the bolt and was heading back to the bolter's cab when EE tripped and fell on a rock, striking EE's knee.
Ee reports getting into the cab of equipment, startled by loud noise, fell into EE's chair. Ee reports unsure how, but EE's knee popped out. EE was diagnosed with a non-occupational condition that contributes to dislocations etc. Following medical investigation, on 9/18/24 cumulative trauma accepted by workers' comp, strained knee, prompting this report.
Ee was adjusting shovel cable. EE lost EE's balance and went to catch self and landed on EE's hand wrong. Seen at ER, diagnosed with a dislocation, and referred for further care. Released with restrictions that site could accommodate. EE's one day of restricted was a paid fatigue day. Ee released to full duty 8/14.
Ee was walking to work area. Ee reports feeling knee pop and having instant pain, then fell to ground. No uneven ground, no work related condition reported. Seen at ER, put off work pending ortho evaluation. WC claim denied, but reporting per MSHA regulation.
Ee reports walking up stairs at work. Felt pop and pain in left calf. Reports inability to bear weight. Put off work pending follow-up on 7/16. On 7/19 determined not compensable by WC, however, reportable under MSHA.
EE started climbing up the ladders and scaffolding platforms to help at the top of the ball mill discharge. EE got to the 4th scaffolding Deck and turned body away from the ladder and towards the deck. EE proceeded to step off and heard a loud pop from EE's left knee as EE stepped to the deck.
Ee was walking with a bucket, reports trip/fall, landed on right shoulder. Diagnosed with contusion to right shoulder, released to restricted duty.
EE working in containment area reports slurry was "thigh high". Trying to walk through it, right foot sank, stepped back, and their left foot stuck. They fell counterclockwise and into a squatting position. Strained right knee. On restrictions site can accommodate.
EE went upstairs to get parts. On the way back down EE remembers feeling like they were going to fall. EE remembers being on the floor, trying to get up. EE reported incident, worked rest of the shift. The next day they asked to see doctor. EE also reported having blood pressure over the past week. Seen at clinic diagnosed with sprain left knee and ankle. Restricted duty.
EE was going to sit in haul truck and misjudged the distance to the seat and fell, hitting their side against the seat. EE was seen at Health, diagnosed with a fractured rib, and put on restrictions site can accommodate.
EE was walking and their foot got stuck in the mud. They tripped and fell forward, landing on both knees and hands. Seen at ER, diagnosed with a strained knee, and released with restrictions site can accommodate.
Ee got off the grader and walked around the equipment to open the drain. EE reports falling forward onto both knees. The ground was wet and snowy. EE felt a pop in the right knee. Seen at ER, x-rays positive for fractured patella, released with restrictions. Ortho consult scheduled for 4/10.
EE was getting out of light vehicle. EE stepped out and into a parking ditch. The ditch was deep, and EE twisted right ankle. Seen at Golden Health 3/24, released with restrictions. X-rays on 3/25 were positive for a small avulsion fracture.
RAM UG Elect team was being escorted from their UG work heading to the cage via UG UTV's. The escort pulled into a muck bay leaving the RAM buggy in direct line with the mucker. Before the buggy was hit the RAM team members jumped out of the buggy, the injured RAM employee fell and slid on the ground scraping hands and bruising left leg and ankle.
EE was getting out of skidsteer and they slipped and fell, landing on their wrist. Seen at clinic, diagnosed with a left wrist distal ulna fracture and a contusion. Placed in hard splint by ortho. Released to restricted duty site can accommodate.
EE felt dizzy so they took a 5 minute break and ate some jerky; then decided to take lunch break, had soup and water. Walked over to garbage following lunch, put water in fridge, blacked out and fell on face (according to witnesses). Cause of syncope unknown. EE fractured maxillary sinus.
