Employee was on break in the personal parking lot. EE was leaning on the rear of another employee's personal pickup. EE turned to the right to walk and tripped over the receiver hitch on the pickup. Employee landed on the sidewalk.
Employee slipped on wet surface in well field, landing on tail bone. Employee reported to Supervisor & continued to work. On 9/12, the employee requested a medical evaluation. Employee was released to work without any restrictions. On 9/19 employee experienced swelling in knee. Employee returned to the doctor and was given 3 days of restricted duty beginning on 09/20/2019.
Employee was working with three others scraping dirt off the front of a dump bed to prepare for liner replacement. EE turned slightly on the metal surface, when EE slipped, fell backwards and automatically reaching out with EE's left hand to catch self. EE felt pain in EE's wrist. EE suffered a fracture in the left wrist.
Mechanic was called to jump start a dozer. After handing the cables to the dozer operator, the employee turned to the other dozer. After taking two steps, the employees left leg slipped under the employee, causing the employees left knee to hit the ground. The area had been skimmed off. Misting rain had begun 3 hours prior with heavy rain forecasted for later.
An Administrative Assistant was walking back up the driveway, returning from the mailbox, carrying the mail in their left arm. After crossing the highway, they stepped on a small rock. It caused them to lose their balance and fall towards the right side. They tried to catch themselves with the right hand. Both the right hand and elbow landed on the concrete.
End of the shift, EE was climbing down a the equipment ladder of a Cat 777 end dump truck. EE didnt use lunch box rope. When EE climbed down to the bumper, EE moved EE's lunch box from the radiator down to the bumper with EE's left hand. When EE started to climb down, EE lost grip with EE's right hand before EE could gain grip with EE's left hand. EE fell approximately five feet.
The injured employee was aiding another mechanic with truck repairs. After completing task, the mechanic went to see if parts were available to change out the fender. The injured mechanic decided to go ahead and remove fender. As EE stepped across the fender, the fender brackets broke. The employee slid down the fender, landing onto the shop floor.
Water bottle placed inside a rolled up Hydraulic hose. Employee chose to step in the middle of the rolled up hoses and stepped on the bottle causing EE to fall to EE's knees rolling EE's left ankle.
Employee was removing the discharge line from an unused 6' water pump. After applying pressure, the hose came loose. The employee lost his balance, falling backwards onto the ground. It was found that sandy materials from the water caused the pipe hard to be removed.
An excavator operator was performing his pre-shift inspection. As he was mounting the 349E Cat excavator, he lost his footing and fell back approximately 1.5 feet landing awkwardly on his left foot spraining his left ankle.
A dozer operator was getting off of a D6T Cat dozer and stepped onto a one foot high wind row and his foot slid down the back side of the wind row causing his right foot to hyperextend.
Employee was standing on dozer track checking transmission oil when he had finished he turned to go to the cab, he twisted right ankle.
Employee stumbled before exiting restroom door. He landed with a closed right hand hitting the bottom of the door jam. Upon investigation, no tripping hazards were found. There were no witnesses to the incident.
Employee was walking from the tractor to his personal vehicle at quitting time and mis stepped causing him to land improperly on his foot. The area was free of all tripping hazard during the time of the incident. When the employee's foot made contact to the ground, it was angled and the force causing a break in his right ankle.
While pulling a length of 3/4" hose, across the ground, the employee lost his balance. He raised his right arm to steady himself and partially dislocated his shoulder. The employee's right hand or arm never came into contact with anything.
EE claimbing into DL tub, stepped on weedburner he had left in floor and twisted ankle. Mildly distracted fracture thru the tip of 5th metatarsal.
Employee was removing a pump (75lbs). from 785 hauling truck. no strap was used to secure to secure the pump. He had oily hands. The pump slipped, striking left side of head knocking him to the ground.
While rolling up a length of 1 1/4 inch rubber hose at a remote fueling location, the mechanic stumbled and fell. While attempting to catch himself with his right hand, a small rock punctured his palm. Closing the wound required three stitches and a tetanus shot was given.
The employee was dismounting a cat d-10-r dozer when he lost his footing and fell to the ground from the blade push arm.
While mounting the crawler type backhoe, the employee slipped from the step to the ground straining his back.
Employee was dismounting that #1 dragline, descending the steps between the house staircase and the top of the shoe. He slipped and fell, striking his elbow and back.
Pulling chocker cable to drill, ground was muddy, ee slipped. He kept on working, did not report being hurt, ran dozer rest of day. Did not call in or report for work 10-23, 10-24-06. Called at 3:45 pm on 10-24-06 and said he had hurt his back.
CONDITIONS WET FOOT SLIPPED FROM STEP AND INJURED LEFT HIP - NO FRACTURES.
EE WAS DESCENDING THE STAIRS OF THE 4BELT/2BELT TRANSFER STATION & AS HE NEARED THE GROUND HE LET LOOSE OF THE HANDRAIL. AS HE STEPPED ON THE 2ND STEP FROM THE BOTTOM A WELD FAILED & ONE SIDE OF THE STEP DROPPED 4" CAUSING HIM TO LOSE HIS BALANCE & FALL APPROX 12" TO THE GROUND
HELPING TO LOAD A ROLL OF FIRE HOSE ON TO A PICK UP TRUCK BALANCE AND FELL OFF THE TAILGATE OF A PICK UP TRUCK LANDING IN AWKWARD POSITION INJURIES HIS LEFT LEG.
INJURED WAS DRAGGING OXYGEN-ACETLYENE CART FROM POWER CENTER AREA TO WORK SITE. HE SLIPPED AND FELL STRKING RIGHT ELBOW AND JAMMING SHOULDER.
SLIPPED UNDERGROUND AND TWISTED KNEE, COMPOUNDING INJURY BY LIFTING ON PANEL.
EE MINER OPERATOR WAS ADVANCING LINE CURTAIN. HE WAS INSTALLING A NAIL THR THE CURTAIN. HE LOST HIS BLANCE AND THE SPAD GOT CAUGHT BETWEEN LITTLE FINGER AND RING FINGER. AS HE FELL THE SPAD CA USED A LACERATION WHICH REQUIRED 12 STITCHES.
GREASING BELTS WHEN HE SLIPPED AND TWISTED LEFT KNEE. HE DID NOT MISS WORK UNTIL MARCH 13, 2003. IT WAS DETERMINED TO BE JOB RELATED APRIL 1, 2003.
COMING DOWN LADDER, FOOT SLIPPED TWISTING RIGHT KNEE.
EE WAS HELPING TWO OTHER MEN LOAD A S/C TIRE INTO THE BED OF A UNIHAULER. HE SLIPPED & FELL ONTO THE FLOOR OF THE UNIHAULER & THE TIRE FELL ON HM. INATTENTION TO ASSURE SOLID FOOTING A FACTOR.
THE INJURED EMPLOYEE WAS CLIMBING THE ACCESS LADDER OF CO #253 DART COAL HAUL TRUCK. HIS RIGHT FOOT SLIPPED OFF THE RUNG OF THE LADDER AND HE CAUGHT HIMSELF WITH HIS LEFT HAND ON THE HANDRAIL. THE TWISTING MOTION AS HE SLIPPED CAUSED A MUSCLE STRAIN TO HIS BACK.
THE INJURED EE WAS PREPARING TO OPERATE A CAT D10-R DOZER IN THE PRE-STRIP FILL AREA. IN CONDUCTING HIS PRESHIFT INSPECTION HE STEPPED ON SOME SPILLED OIL ON THE UPPER DECK, SLIPPED AND FELL O NTO THE DOZER TRACKS BRUISING MUSCLES IN THE LOWER BACK LUMBAR AREA.
BENDING TO PICK UP FIRE EXTINGUISHER, SLIPPED IN OIL ALLEGED LOST TIME ACCIDENT STILL BEING INVESTIGATED.
THE INJURED EE WAS DISMOUNTING A CAT D-7 DOZER. AS HE STEPPED FROM THE TRACK ONTO THE BLAD PUSH ARM, HIS FOOT SLIPPED OFF OF THE PUSH ARM STEP ALLOWING HIS RIGHT SHIN TO CONTACT (SCRAPE) AGAIN ST THE TRACK PAD. THE ORIGINAL INJURY WAS A CONTUSION/ABRASION APPROX. 3" LONG. 2 1/2 WEEKS POST INJURY THE EE HAS A MAJOR INFECTION OF THE INJURED AREA REQUIRING I-V ANTIBIOTICS (OUTPATIENT)
EE WAS CROSSING A CONVEYOR BOLT THAT WAS OUT OF SERVICE. WHEN HE PLACED ONE OF HIS FEET ON A BELT RAIL AND STARTED TO STEP ACROSS HIS FOOT SLIPPED AND HE FELL, STRIKING THE BELTRAIL. THIS RESU LTED INTO A CONTUSION TO HIS RIGHT THIGH. EE MISSED NO WORK HOWEVER ON 6-29-01 HE HAD SOME FLUIDS DRAINED FROM THAT THIGH.
EE WAS WALKING IN THE NO. 2 ENTRY OF THE C-R LONGWALL SECTION WHEN HE SLIPPED WHERE SOME MUD AND WATER WERE PRESENMT. AS THE RESULT OF SLIPPING HE TWISTED HIS BACK FOR WHICH CHIROPRACTOR TREAT MENTS ARE BEING RECEIVED.
EMPLOYEE WAS CARRYING A BAG OF BLOCK BOND. HIS RIGHT FOOT CAUGHT ON A PLASTIC BAND CAUSING EMPLOYEE TO STUMBLE AND FALL. EMPLOYEE'S LEFT FOOT TWISTED WHEN HE FELL RESULTING IN STRAINED LIGAMEN TS AND A POSSIBLE CHIP FRACTURE IN HIS LEFT FOOT
WHILE TIGHTENING UP A 50-TON ROOF JACK HIS LEFT FOOT SLIPPED CAUSING PAIN AND SWELLING IN LEFT KNEE.
GARDNER WAS CLIMBING THE STEPS TO GO INTO THE CAB OF THE D9R DOZER THAT HE WAS GOING TO OPERATE.WHILE DOING THIS HE SLIPPED WHICH CAUSED A LOT OF HIS BODY WEIGHT TO BE SHIFTED BY HIS RIGHT AR M. THIS RESULTED INTO A STRAIN TO HIS RIGHT ELBOW. THIS HAS NOW BEEN DIAGNOSED AS A MUSCLE TEAR AND HE RECENTLY BEGAN TO MISS WORK DUE TO THIS PROBLEM.
EE WAS CROSSING THE TRACK AT THE 3NE JEEP SHOP. HE TRIPPED ON THE RAIL AND FELL ON HIS RIGHT SIDE. A DR.'S EXAMINATION ON 3-16-2001 REVEALED THAT EE HAD A HERNIA ON THE RIGHT SIDE OF HIS GROIN .
EE WAS WALKING DOWN TRACK AND STEPPED ON A CROSSTIE WHICH WAS UNDER WATER AND TWISTED HIS RIGHT KNEE. HE CONTINUED WORKING UNTIL JUNE 6 2001 WHEN HE TOOK OFF TO HAVE.
EE WAS WALKING DOWN THE NO.3 ENTRY, OF THE C-3 LONGWALL TEARDOWN, LOOKING FOR SUPPLIES. WHILE DOING THIS HE STEPPED IN A HOLE IN THE MINE FLOOR TWISTING HIS BACK. THIS INJURY WAS NOT REPORTED UNTIL 2-13-01 EE INFORMED US THAT HE HAD STARTEDCHIROPRACTO TREATMENT FOR THIS INJURY.
CARRYING BELT STAND TO XCUT, STEPPED ON UNEVEN BOTTOM TWISTING LEFT KNEE. LAST DAY WORKED: 03/08/01.
EE WAS HELPING RECOVER BELT STRUCTURE. HE TURNED TO DISCONNECT A SECTION OF STRUCTURE. HE TRIPPED AND FELL ON HIS RIGHT ARM, FRACTURING IT.
EMPLOYEE WAS CROSSING THE SPILL TRAY OF THE LONGWALL FACE CONVEYER TO CHANGE BITS IN THE HEAD DRUM OF THE SHEARER, WHEN HE SWUNG HIS LEFT LEG OVER THE SPILL TRAY HE EXPERIENCED PAIN IN HIS LOW ER BACK EXTENDING DOWN INTO HIS LEFT HIP AND LEG
WHILE ROCKDUSTING 10 BELT WITH HOSE, HE WAS WALKING BACKWARDS AND STEPPED ON A SMALL BOTTOM ROOLER WHICH CAUSED HIM TO TWIST HIS LEFT KNEE AND FALL DOWN.
EMPLOYEE WAS PREPARING TO GET OUT OF THE OPERATORS DECK OF A MANTRIP WHEN HE SLIPPED AND FELL, INJURING HIS BACK. THIS INJURY HAS BEEN DIAGNOSED AS A LUMBAR STRAIN. (SEE ATTACHED LETTER.)
EE HAD JUST GOTTEN OUT OF THE MANTRIP AND WAS WALKING TOWARD THE 7 LEFT SECTION WHEN HE STEPPED ON SOME UNEVEN BOTTOM AND SLIPPED, CAUSING HIM TO EXPERIENCE SOME PAIN IN HIS RIGHT KNEE. THIS W AS LATER DIAGNOSED AS A TORN MENISCUS FOR WHICH SURGERY WAS PERFORMED ON 12/14/00. FIRST DAY WAY FROM WORK 12/13/00.
THE EMPLOYEE WAS WALKING ALONG THE 39 BELT AT BLOCK 193, CONDUCTING A PRE-SHIFT EXAMINATION. HE SLIPPED AND TWISTED HIS LEFT KNEE.
