IE was using a tag line for a suspended load when EE stumbled over a traffic cone causing EE to fall to the ground injuring left ribs and elbow. After medical evaluation, IE had no broken bones but did receive one suture in the left elbow. 2 days later IE was having issues with pain, went to ER and confirmed 3 broken ribs.
Employee was walking to the parts car in 9 South Mains, when EE tripped causing EE's left knee to strike against a loose rock. Diagnosed with a torn meniscus. LTA as of 11/20/24.
EE was working on the #1 West Section helping the electricians install a bottom pin on the 002 C/M that was located in the intersection of the #6 entry near spad #2311. EE stepped of the miner slipped twisting EE's left knee. Please note the accident occurred 10/30/24 EE could not see a specialist until January 6, 2025 and it was determined by the Doctor EE will be off work.
EE was working on the #1 North Section in entry #7 L.O.B. carrying cribs and fell twisting their left hand. Please note the employee was supposed to return to work on 10/12/24 per Dr. Excuse. However, EE missed worked on Saturday unrelated to the injury.
Employee was walking along 3 Mother Belt when EE slipped and caught self before falling with EE's left arm grabbing the belt bed rail and felt a strain in EE's left bicep. On 9/4/24, MRI results determined employee has a torn labrum, and bicep tendon.
Employee was working on the roof of the coal sampler building 10' wide x 16' long, to remove a hydraulic cylinder from an auger boom. Employee moved to the edge of the roof and stepped on a glass roof and falling down to their waist. They were secured with a safety harness falling approximately 3'.
Employee was fire bossing slope belt 50' outby tail slope when employee lost EE's footing causing EE's left knee to bend back and underneath self while falling. The employee suffered a knee sprain and tear of EE's quadricep tendon. No other part 50.20-5 apply.
EE was walking on tight side of belt at 7 wall 2K long wall and went to step over coal accumulation and a rock that was about 2ft. wide and as EE was slipping EE caught EE's left foot under the rock and twisted EE's left knee.
Employee stepped through a man door and twisted EE's right knee. Employee was diagnosed with a fracture to EE's patella and was taken off work for rehab.
Employee was walking in 9 South Mains, when EE's right foot tripped on the mine floor causing EE to fall against the rib. EE was diagnosed with a torn Achilles tendon and fracture to the fifth metatarsal in the right foot.
Employee was carrying a crib bag on EE shoulder and, when they went to drop it, it caught on their mine belt twisting EE left knee. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
Employee was carrying 2 cans of hydraulic oil; employee slipped catching self, causing a strain to the lower back. Note: Employee did not seek medical attention until 04/17/2024.
Employee tripped on uneven bottom and fell to the ground catching self with left hand on a sharp rock. This caused a laceration on EE's left hand requiring sutures to close. Employee did not miss scheduled work time due to this injury.
Employee was carrying and spotting J-Crib bags in the intake, when EE lost their balance and struck the right knee off of the ground. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
Employee was walking #4 Mother Belt conducting a pre-shift examination when they slipped on mud. From this event they fell to the ground and while falling EE attempted to brace themselves against the ground with right arm, which caused right shoulder to be injured from this action. Diagnosed with a torn rotator cuff on 12/5/23.
While walking to re-hook a chain onto a track rail that was being removed from 11L longwall, the employee tripped and fell onto the track rail. EE's knee landed on the on the track rail. Employee worked all required shifts until being taken off of work for a fractured kneecap on 9/11/2023.
Employee tripped on water hose while washing the floor in the foreman's locker room. They landed on their right side and experienced a cut on knee and bruise on elbow. Employee stated that whole right side of body hurt.
Employee was walking thru a crosscut, tripped and accidentally struck hand against a plate of a cable bolt that was on the ground causing injury to hand.
Employee threw a track switch. As EE was backing up to a manhole to clear the track to allow a mantrip to pass, employee stepped on a loose piece of coal and fell, twisting right ankle.
Employee was working on the surface area of an underground mine when employee tripped over a 2" hose and fell. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
Employee was rib bolting on the miner in the 3B section when they went outby to get a piece of wire mesh. As they pulled the mesh inby, they tripped on a rock and sprained their left ankle.
Injured employee got out of cab of scoop and walked toward the bucket end. IE stepped in a hole and tripped, landing on their right arm, dislocating right shoulder. Injury was not the result of a training or compliance issue.
Mains unit #1 Section - employee was getting on a ride and twisted knee
Employee tripped and fell while walking past the loadcenter in 2K section. Employee used right arm to catch self and felt a pain in right elbow. Employee was diagnosed with a fractured Distal Humorous (small bone in elbow)
While walking outby from the face past the loading machine, employee tripped on the cables and fell to the ground, landing on right knee and shoulder.
Employee was walking to throw the 2 East Mains switch, while employee was walking the employee rolled an ankle. Resulting in an avulsion fracture. It didn't involve job skills, proficiency, training, or attitude. No PPE issues involved. No other part 50 criteria applies.
1 North Panel near spad 391 - while removing a regulator employee tripped and fell onto a concrete block, then a portion of the remaining regulator fell onto employee's back.
Employee was walking out of the 12J section, left foot stepped in a shuttle car rut causing employee to slip and fall. Employee felt pain in left ankle. Employee was subsequently diagnosed with a fractured bone in the ankle.
Truck driver was putting water in truck. They climbed up on the tire. When climbing down they slipped and fell twisted a knee.
Employee was traveling along a belt drive and stepped on a roller causing them to slip and fall onto the roller causing injury to an arm.
Employee was side bolting in the 12J Section, as EE was retrieving 6 foot bolts from on top of the rear of the miner EE's right foot slipped off the step. EE's foot got caught in the miner cable and EE felt a pain in EE's right ankle. EE continued to work all EE's scheduled shifts until EE began missing work on 2/25/2022.
Employee was walking around the tail of the slope and EE stepped wrong and rolled ankle causing injury.
While hosing in the raw coal transfer building, employee slipped on wet floor and struck leg against a kick plate, causing a laceration. Injury was not the result of a training or compliance issue.
Employee was walking down the pan line in 8L Longwall Section, when EE slipped. EE felt a pain in EE's left shoulder when EE caught self. Employee reported that the pain has came back over the last couple of weeks. EE worked all scheduled shifts between October 10th 2021 until February 17th 2022 when EE was taken off of work due to the injury to EE's left shoulder.
While cleaning in Silo #4, employee slipped in grease and felt pain in knee and back. Injury was not the result of a compliance or training issue.
Employee was placing spreaders on bottom forms when the one they were standing on twisted causing them to fall four feet.
WENT TO WALK ACROSS TRACK TO TIGHT SIDE, TRIPPED OVER RAIL, TWISTED LEFT KNEE.
Employee lost balance after turning on a water line and struck back against a handrail causing a strain. Injury was not the result of a training or compliance issue. Employee did not start missing work until 8/26/2021.
Employee tripped on track tie and struck knee on a spike puller causing injury.
Employee was standing on a bar grating platform to hose off floor steel when the grating gave way causing employee's right foot to fall through a hole in the platform.
Employee was mucking at the Enlow 2 South #1 Airshaft Site. During the process of switching lifting hooks from one muck bucket to another employee slipped on the wet shale and rock. This caused a rolling of the right ankle beyond normal range of motion.
Employee was walking to the mantrip when EE stepped into a hole and felt a pain in right ankle. Resulting in a sprained ankle and a small avulsion fracture.
While operating the Longwall shearer employee tripped and fell onto right arm. Employee returned to work next scheduled shift and worked all required shifts until being taken off for surgery to repair shoulder on 10/6/2021. Lost work days will be counted from 10/6
Employee's right foot got caught on a piece of mesh causing EE to trip. Resulted in a fracture tibia / chip fracture on base of tibia.
EE was pushing the drill rig arm forward while setting a collar pipe in the hole EE just drilled. EE's foot slipped and right shoulder popped out of place. EE had the collar pipe in left hand and was pushing the rig with right hand when EE slipped. As EE slipped and caught self EE's right shoulder became dislocated.
Employee was recovering vent tubes in the 5B HG section from 4 Left entry. While pulling a tube up off the mine floor, EE lost balance and fell towards the rib. While falling forward EE stuck right hand out to brace self, when EE's hand struck against a rib strap. Diagnosed with a fracture to the right 5th finger on 2/10/21.
The employee had just arrived at the Dry Ridge portal and was walking from their vehicle to the bath house when they slipped on ice, falling and landing on left elbow. X-rays would later reveal a fracture of the ulna bone.
Employee was walking to their car to retrieve an item - slipped on a rubber mat over a metal grating and fell to left knee. Small laceration and visible bruising to knee area
Employee was standing on the toes of a shield to advance the face, lost balance and fell backwards and right knee twisted causing pain. Diagnosed with a tibial plateau fracture and torn ACL / meniscus on 11/9/20.
Employee was carrying a wing roller to change out. EE was carrying the roller up a set of steps and on the last step EE tripped and fell on the landing and the roller landed on EE's left thumb, resulting in fracturing EE's thumb and lacerated it also. EE was carrying the roller on EE's right shoulder.
