EE WAS INSPECTING UNIT FACE WHEN HIS FOOT CAUGHT BETWEEN TWO PIECES OF 18 INCH TUBING CAUSING HIM TO FALL. ON 7/15/03 A POSITIVE MRI REPORT(X-RAY) WAS RECEIVED INDICATING DAMAGE TO KNEE.
EE WAS PERFORMING A PRE-OP INSPECTION WHEN HIS FOOT CAUGHT ON A CABLE, CAUSING HIM TO TWIST HIS RIGHT KNEE. EE DID NOT STUMBLE OR FALL. ON 6-24-03 MEDICAL DIAGNOSIS WAS RECEIVED INDICATING THA T DAMAGE IS LIKELY RELATED TO THIS INCIDENT, NO PAST KNEE SURGERY.
EE SLIPPED ON WET GROUND AND FELT PAIN IN HIS LEFT KNEE.
IN THE COURSE OF PERFORMING HIS WORK TASK, EE FELL FROM A 6-FOOT LADDER ONTO THE MINE FLOOR.
EE WAS PULLING ON A 1.5" WATER HOSE WHEN HE LOSTHIS BALANCE AND FELL BACKWARD TO THE GROUND.
EMPLOYEE WAS WALKING ALONG LONGWALL FACE WHEN HE STEPPED IN MUD AND FELT PAIN IN HIS RIGHT KNEE. EMPLOYEE CONTINUED TO WORK AND ON 3/1/03 STARTED MISSING WORK PRIOR TO HAVING SURGERY ON 3/3/03 TO HIS RIGHT KNEE.
EMPLOYEE WAS ON HIS EXAMINATION ROUTE AND WAS WALKING DOWN THE INCLINED WOODEN RAMP WHEN HE STARTED TO SLIP AND CAME DOWN HARD ON HIS LEFT HEEL. EMPLOYEE WAS ASSIGNED TO RESTRICTED WORK DUTY.
EE WAS WALKING ACROSS A PLYWOOD BRIDGE UNDER THE NORTH SHAFT. THE PLYWOOD WAS SET FROM DRIPPING WATER AND EE SLIPPED, FALLING ON HIS BACK.
EMPLOYEE STEPPED THROUGH A 2 FT BY 5 FT PIECE OF FLOOR GRATING THAT HE HAD PREVIOUSLY MOVED DURING THE SHIFT.
EE STEPPED ON ELECTRICAL CABLE AS HE WAS CROSSING IT WHEN IT TIGHTENED AND CAUSED EE S RIGHT LEG TO BE PULLED BACK AS HE FELL. EE WAS PLACED ON RESTRICTED DUTY.
EMPLOYEE WAS WALKING BELTLINE WHEN HE CROSSED A BRIDGE AND SLIPPED, FALLING TO ONE KNEE.
EE WAS ATTEMPTING TO MOUNT DOZER TO ENTER THE EQUIPMENT CAB. HIS RIGHT FOOT WAS ON FIRST STEP ON SIDE OF CAT TRACK AND HIS HIS LEFT FOOT WAS ON TOP OF CRAWLER PAD. HIS RIGHT FOOT SLIPPED OFF S TEP AND ALL OF HIS WEIGHT TRANSFERRED TO HIS LEFT LEG. EE WAS DIAGNOSED WITH A PULLED LIGAMENT IN HIS LEFT UPPER LEG.
EMPLOYEE WAS STEPPING OFF THE BACK OF A ROOF BOLTER WHEN HE FELT PAIN IN HIS LOWER BACK. RAMP HEIGHT ON BOLTER WAS 20 INCHES FROM THE MINE FLOOR. EMPLOYEE WAS ASSIGNED TO RESTRICTED WORK ACTIV ITY.
EMPLOYEE WAS DESCENDING LADDER WHEN HE CAUGHT HIS BOOT ON A RUNG, SLIPPED & FELT PAIN IN HIS LEFT SHOULDER. SURGERY WAS PERFORMED ON 10/15/02. HE STARTED MISSING WORK AT THIS TIME.
EE WAS INSTALLING LINE CURTAIN. WHILE TRYING TO STAPLE CURTAIN ON RIB, HE STEPPED IN A 6-INCH HOLE AND TWISTED HIS LEFT KNEE.
EE WAS CLIMBING BETWEEN LONGWALL SHIELDS AND PANLINE WHEN HE SLIPPED OFF FRONT OF SHIELD TOE ANDTWISTED HIS RIGHT KNEE. STARTED MISSING WORK ON 1/10/02.
EMPLOYEE BUMPED HIS LEFT ELBOW ON CORNER OF CLA CAB AS HE WAS ENTERING CAB. EMPLOYEE SOUGHT MEDICAL ATTENTION AND HAD FLUID DRAINED FROM HIS LEFT ELBOW.
EE WAS CLIMBING STEEL STEPS WHEN THE MATATARSAL GUARD ON HIS WORKBOOT CAUGHT ON THE BOTTOM STEP,CAUSING HIM TO TWIST HIS LOWER BACK.
EE WAS WLAKING TO TAILGATE WHEN HE STEPPED IN RUT THAT WAS SLIPPERY FROM WATER/MUD AND COVERED WITH ROCK DUST. EE SLIPPED AND FELL BACKWARDS ONTO HIS LEFT AKNLE CAUSING FRACTURE TO ANKLE.
EMPLOYEE WAS WALKING IN DRIVEWAY TO DE-ICING SHED WHEN HE SLIPPED AND FELL ON ICE, LANDING DIRECTLY ON HIS RIGHT ELBOW. EMPLOYEE WAS DIAGNOSED WITH A HAIRLINE FRACTURE AND POSSIBLE BONE CHIP. HIS RIGHT ARM WAS PUT IN A CAST AND HE RETURNED TO WORK WITH RESTRICTION OF LEFT HANDED WORK ONLY.
EE WAS WALKING TO SET A JACK IN THE FIRST-NORTH INTAKE WHEN HE FELT PAIN IN HIS RIGHT KNEE. EE IS CURRENTLY WORKING ON RESTRICTED DUTY. DELAY INSUBMITTING THIS REPORT IS DUE TO INVESTIGATING D ETAIL SURROUNDING THIS INCIDENT.
AT THE START OF SHIFT, EE WAS REPLACING A SPLICE ON THE 1ST NORTH BELT. LATER IN THE SHIFT HE FELT PAIN IN HIS KNEE AND PUT ON A KNEE BRACE. EE THOUGHT HIS KNEE WAS INJURED WHEN HE REACHED FOR A TOOL AND TWISTED HIS RIGHT KNEE DURING THE SLICE REPAIR. *FIRST DAY MISSED 9/14/00 DUE TO SURGERY TO REPAIR MENISCUS TEAR.
EMPLOYEE STATED THAT HE BECAME DIZZY AND FELL OUT OF A MANTRIP.
