Employee was walking down steps of trailer when he missed a step and came down hard on his left knee, causing pain in his knee.
Employee was hanging curtain when he blacked out for a second and fell backwards, injuring his back.

Employee continued working until 9/18/07 when he sought medical attention and began missing work. On 10/22/07 information was received on work-relatedness of this absence.

Employee was walking down entry when he tripped on a tire mark and fell on his left shoulder. The area was very soft from a water event. Employee continued working until 12/12/06 when he had surgery performed on his shoulder and began missing work. Information was received on 12/21/06 relating surgery to this incident.
Employee was pulling slack on belt with channel locks to pin belt together when his foot slipped and he felt pain in his right knee. He sought medical attention on 8/17/06 and was prescribed restricted work duties.
Employee was hand dusting on belt when he felt a sting and burning sensation in his left knee. Employee continued working until 11/21/06 when he started missing work.
Employee was dismounting D8 dozer after checking the oil. He grabbed handhold with right hand and as he stepped back on platform, his foot slipped off causing his right shoulder and hand to be pulled hard. Employee worked until 9/25/06 when he had shoulder surgery and started missing work days.
Employee was exiting door after changing filter at fuel tank when the metatarsal guard on his boot caught on bottom of door and he fell striking his forehead, left knee and tooth. He started missing work on 2/3/06 due to back injury. Medical verification was received on 2/20/06 on work relatedness of his back injury to this claim.
Ground was frozen, snow had an accumulation of 2 inches. EE was exiting his vehicle at the N.E. holding pond to start a pump. He had traveled approx. 10 ft and slipped. He broke his left fibula and damaged ligaments in his left ankle that requires surgery to insert 2 pins to stablize his ankle.
Employee was walking in work environment when he twisted his right foot. Information was received on 10/17/05 confirming the work-relatedness of the foot injury.
Employee was washing floor in shower area using a garden hose when hose caught on bench leg. As employee tried to free hose, he slipped and twisted his left knee. On 12/1/05 employee had surgery to repair knee and began missing work at that time.
Employee was pulling pod dust hose to assist co-worker who was spreading dust at end of hose. As employee stepped backwards in sideways position, he felt weakness in his left knee. He continued working until 5/16/05 at which time he started missing work.
Employee was stepping on the floor after running a cable under the drive pulley of belt in prep plant when his right ankle twisted.
Employee was walking down stairs at bunker when he missed the last step and jammed his left knee. He continued working on regular duty until 6/2/04 when he was diagnosed with medial meniscus tear in his left knee and assigned to restricted duty pending surgery.
Employee was helping to carry full piece of roof mesh when his left foot became tangled in wire. He attempted to pull out his foot, twisted his knee and fell striking his left knee on the ground.
Employee was pulling 60" bottom roller under belt through mud when rope slipped off and he fell on his left shoulder and knee. On 11/2/05 employee had surgery to repair his left knee and began missing work at that time. Shoulder surgery is scheduled for 11/23/05.
EE WAS OPERATING LONGWALL SHEARER AND AS HE STEPPED INTO THE NEXT LONGWALL SHIELD, HE LOST HIS FOOTING AND FELL ON HIS LEFT ARM CAUSING CONTUSION TO HIS LEFT SHOULDER.
WHILE REACTING TO A LEAKY COMPRESSED AIR LINE, EMPLOYEE FELL LANDING ON HIS ARM AND SHOULDER.
EMPLOYEE WAS SITTING IN RAMP ON SURFACE PRIOR TO HER SHIFT WHEN SHE FAINTED AND FELL, STRIKING HER FACE ON THE CONCRETE FLOOR. SHE RECEIVED SUTURES TO HER LIP, JUST ABOVE HER NOSE AND ABOV E HER LEFT EYE.
EE WAS INSPECTING UNIT FACE WHEN HIS FOOT CAUGHT BETWEEN TWO PIECES OF 18 INCH TUBING CAUSING HIM TO FALL. ON 7/15/03 A POSITIVE MRI REPORT(X-RAY) WAS RECEIVED INDICATING DAMAGE TO KNEE.
EE WAS PERFORMING A PRE-OP INSPECTION WHEN HIS FOOT CAUGHT ON A CABLE, CAUSING HIM TO TWIST HIS RIGHT KNEE. EE DID NOT STUMBLE OR FALL. ON 6-24-03 MEDICAL DIAGNOSIS WAS RECEIVED INDICATING THA T DAMAGE IS LIKELY RELATED TO THIS INCIDENT, NO PAST KNEE SURGERY.
EE SLIPPED ON WET GROUND AND FELT PAIN IN HIS LEFT KNEE.
IN THE COURSE OF PERFORMING HIS WORK TASK, EE FELL FROM A 6-FOOT LADDER ONTO THE MINE FLOOR.
EE WAS PULLING ON A 1.5" WATER HOSE WHEN HE LOSTHIS BALANCE AND FELL BACKWARD TO THE GROUND.
EMPLOYEE WAS WALKING ALONG LONGWALL FACE WHEN HE STEPPED IN MUD AND FELT PAIN IN HIS RIGHT KNEE. EMPLOYEE CONTINUED TO WORK AND ON 3/1/03 STARTED MISSING WORK PRIOR TO HAVING SURGERY ON 3/3/03 TO HIS RIGHT KNEE.
EMPLOYEE WAS ON HIS EXAMINATION ROUTE AND WAS WALKING DOWN THE INCLINED WOODEN RAMP WHEN HE STARTED TO SLIP AND CAME DOWN HARD ON HIS LEFT HEEL. EMPLOYEE WAS ASSIGNED TO RESTRICTED WORK DUTY.
EE WAS WALKING ACROSS A PLYWOOD BRIDGE UNDER THE NORTH SHAFT. THE PLYWOOD WAS SET FROM DRIPPING WATER AND EE SLIPPED, FALLING ON HIS BACK.
EMPLOYEE STEPPED THROUGH A 2 FT BY 5 FT PIECE OF FLOOR GRATING THAT HE HAD PREVIOUSLY MOVED DURING THE SHIFT.
EE STEPPED ON ELECTRICAL CABLE AS HE WAS CROSSING IT WHEN IT TIGHTENED AND CAUSED EE S RIGHT LEG TO BE PULLED BACK AS HE FELL. EE WAS PLACED ON RESTRICTED DUTY.
EMPLOYEE WAS WALKING BELTLINE WHEN HE CROSSED A BRIDGE AND SLIPPED, FALLING TO ONE KNEE.
EE WAS ATTEMPTING TO MOUNT DOZER TO ENTER THE EQUIPMENT CAB. HIS RIGHT FOOT WAS ON FIRST STEP ON SIDE OF CAT TRACK AND HIS HIS LEFT FOOT WAS ON TOP OF CRAWLER PAD. HIS RIGHT FOOT SLIPPED OFF S TEP AND ALL OF HIS WEIGHT TRANSFERRED TO HIS LEFT LEG. EE WAS DIAGNOSED WITH A PULLED LIGAMENT IN HIS LEFT UPPER LEG.
EMPLOYEE WAS STEPPING OFF THE BACK OF A ROOF BOLTER WHEN HE FELT PAIN IN HIS LOWER BACK. RAMP HEIGHT ON BOLTER WAS 20 INCHES FROM THE MINE FLOOR. EMPLOYEE WAS ASSIGNED TO RESTRICTED WORK ACTIV ITY.
EMPLOYEE WAS DESCENDING LADDER WHEN HE CAUGHT HIS BOOT ON A RUNG, SLIPPED & FELT PAIN IN HIS LEFT SHOULDER. SURGERY WAS PERFORMED ON 10/15/02. HE STARTED MISSING WORK AT THIS TIME.
EE WAS INSTALLING LINE CURTAIN. WHILE TRYING TO STAPLE CURTAIN ON RIB, HE STEPPED IN A 6-INCH HOLE AND TWISTED HIS LEFT KNEE.
EE WAS CLIMBING BETWEEN LONGWALL SHIELDS AND PANLINE WHEN HE SLIPPED OFF FRONT OF SHIELD TOE ANDTWISTED HIS RIGHT KNEE. STARTED MISSING WORK ON 1/10/02.
EMPLOYEE BUMPED HIS LEFT ELBOW ON CORNER OF CLA CAB AS HE WAS ENTERING CAB. EMPLOYEE SOUGHT MEDICAL ATTENTION AND HAD FLUID DRAINED FROM HIS LEFT ELBOW.
EE WAS CLIMBING STEEL STEPS WHEN THE MATATARSAL GUARD ON HIS WORKBOOT CAUGHT ON THE BOTTOM STEP,CAUSING HIM TO TWIST HIS LOWER BACK.
EE WAS WLAKING TO TAILGATE WHEN HE STEPPED IN RUT THAT WAS SLIPPERY FROM WATER/MUD AND COVERED WITH ROCK DUST. EE SLIPPED AND FELL BACKWARDS ONTO HIS LEFT AKNLE CAUSING FRACTURE TO ANKLE.
EMPLOYEE WAS WALKING IN DRIVEWAY TO DE-ICING SHED WHEN HE SLIPPED AND FELL ON ICE, LANDING DIRECTLY ON HIS RIGHT ELBOW. EMPLOYEE WAS DIAGNOSED WITH A HAIRLINE FRACTURE AND POSSIBLE BONE CHIP. HIS RIGHT ARM WAS PUT IN A CAST AND HE RETURNED TO WORK WITH RESTRICTION OF LEFT HANDED WORK ONLY.
EE WAS WALKING TO SET A JACK IN THE FIRST-NORTH INTAKE WHEN HE FELT PAIN IN HIS RIGHT KNEE. EE IS CURRENTLY WORKING ON RESTRICTED DUTY. DELAY INSUBMITTING THIS REPORT IS DUE TO INVESTIGATING D ETAIL SURROUNDING THIS INCIDENT.
AT THE START OF SHIFT, EE WAS REPLACING A SPLICE ON THE 1ST NORTH BELT. LATER IN THE SHIFT HE FELT PAIN IN HIS KNEE AND PUT ON A KNEE BRACE. EE THOUGHT HIS KNEE WAS INJURED WHEN HE REACHED FOR A TOOL AND TWISTED HIS RIGHT KNEE DURING THE SLICE REPAIR. *FIRST DAY MISSED 9/14/00 DUE TO SURGERY TO REPAIR MENISCUS TEAR.
EMPLOYEE STATED THAT HE BECAME DIZZY AND FELL OUT OF A MANTRIP.
