EE was throwing salt into the bed of his trailer. EE was inside of salt building getting salt out of the barrel, turned to throw the salt and stepped on loose salt on the floor. Foot slipped and EE tore a muscle in his lower back.
Walking to truck, slipped & fell in mud. Bruised ribs. Went home, changed clothes & came back to work. Hauled 2 loads of coal, complained about ribs hurting. Went to United Hospital Center & got doctor's excuse to take rest of day off work. Called in next day & said he was too sore to work. Returned to work next day.
It is beleived that employee had a seizure, fell and struck the back of his head on 3" gravel. Doctors excuse ran from 6/25/08 thru 6/27/08. He was not allowed to return to work until 7/1/08 pending a drug test and results by orders of company official.

Employee was walking down a snow covered area towards pit mouth to put scoop on charge when he fell on a piece of sheet metal that was covered with snow, injuring his left hip.
OPERATOR WAS CLIMBING DOWN FROM CATERPILLAR 930 END LOADER AND SLIPPED OFF THE STRP IN THE POURING RAIN. HE LANDED ON FENDER HITTING HIS RIBS. HE WORKED THE 4 DAYS, 7-12-04 TO 7-15-04 BEFORE HE WENT TO THE DOCTOR.
5NW SECTION OUTBY FEEDER IN #6 ENTRY. EMPLOYEE GETTING OUT OF SCOOP (OPERATORS COMPARTMENT), NO CANOPY, HE STOOD UP & HIS FOOT SLIPPED ON MUD & OIL ON THE DECK & HE FELL BACKWARDS. HIS BACK HIT CORNER OF SCOOP AS HE FELL OUT OF THE DECK. POSSIBLE LOWER BACK INJURIES.
Ee standing in operator's compartment on operator's seat, lifted left leg to step on top of machine. The seat was not attached and slipped from under his weight. He fell backwards hitting his right side on edge of operator compartment.
THE EMPLOYEE SHUT THE BELT OFF AND CROSSED THE TAIL PIECE TO GETA SHOVEL AND CAME BACK ACROSS THE TAIL PIECE AND FELL AND HIT HIS KNEE.

WALKING INTO SHOWERHOUSE AND TRIPPED. SHE WENT TO CATCH HERSELF AND BRUISED HER HAND.
THE EMPLOYEE WAS DRAGGING A PIECE OF LINE CURTAIN AND TRIPPED AND FELL AGAINST A SHUTTLE CAR.
DRIVING DIESEL MANTRIP ON STEEP GRADE, ENGINE STALLED ON HILL AN DHT EBRAKES FAILED. MANTRIP RAN DOWN HILL TOWARDS PARKED SCOOP OEPRATOR JUMPED FROM MANTRIP LANDING ON HIS RIGHT SHOULDER.
THE EE WAS GETTING OUT OF THE ENDLOADER & STEPEDON A ROCK & TWISTED HIS ANKLE
THE EE STATED THAT HE WAS REPLACING ROOF BOLTS THAT HAD BEEN CUT OUT BY THE CONTINUOUS MINER ANDWAS TRYING TO CATCH HIMSELF TO KEEP FROM FALLING AND CAUGHT A ROOF BOLT PLATE.
