EE exited EE trailer when he slipped on a patch of ice on the ground. He landed on his left side. X-rays of shoulder negative, placed on restrictions w/1st follow up to be on 1/24/14. RTW on 24th and L hand was swollen, X-ray was negative. Placed on restrictions: light finger work only with L hand and no operating company vehicles. Follow up apt 1/31. Reported due to restrictions.

Showing all 8EE stated he was in the process of turning the fire hydrant on to fill the steam cleaner. As he was walking next to the hydrant he tripped over the main shut off valve pipe that extended 6" above ground level He fell on his left shoulder. The injury became reportable on 8/1 when he was issued a prescription medication and released to work with restrictions.
Employee was dismounting anfo truck when he stepped on uneven ground causing a twisting motion to his right ankle. This resulted in a sprain. He returned to work with restrictions. Limit standing/walking to 15 min per hour 2 hours per chift.
Employee stated he was in the process of spreading a tarp over the 1800 Hitachi Shovel car-body to keep the rain off the area where the disassembly with was being performed. As he took a step to spread the tarp he slipped on the slick surface causing him to fall backwards striking the middle of his back area on the shovel's roller.
EE stated he was cleaning front windshield on loader, when he slipped and fell on his left side(same level), causing a sprain to the left shoulder. EE said that mud accumulated on his boots, which caused the accident.
EE ALLEGES HE SLIPPED ON THE BOTTOM STEP OF #2664 VOLVO TRUCK WHILE MOUNTING TO BEGIN HIS SHIFT. HE COMPLAINED OF PAIN IN THE RT. FOOT & WAS DIAGNOSED WITH A SPRAIN. HE WAS RELEASED TO WORK WITH RESTRICTIONS.
EE STATED THAT WHILE DISMOUNTING THE MAINTENANCE LUBE TRUCK, HIS FOOT SLIPPED OFF THE BOTTOM STEP OF THE OPERATORS CAB CAUSING HIM TO FALL TO THE GROUND. HE STRUCK HIS LEFT ELBOW ON THE GROUND RESULTING IN A CONTUSION. BY DR.'S ORDERS HE WAS RESTRICTED FOR ONE DAY.
EMPLOYEE WAS STEAMING OFF THE CONCRETE FLOOR OF THE OIL CONTAINMENT AREA. HE APPARENTLY SLIPPED BUT DID NOT FALL ON THE SLICK SURFACE AND STRAINED HIS RIGHT KNEE. X-RAYS WERE NEGATIVE. THE EMP LOYEE HAS HAD SURGERY ON THE RIGHT KNEE FOR AN UNRELATED WORK INJURY.
