Showing all 12Employee was getting oil for equipment when he slipped on the ice, he caught himself on the tailgate of his truck but he twisted his lower back & hip.
Was walking from shop area to the fuel truck to start it up, area was icy causing the employee to fall onto his back and injuring his left arm.
Working on grease bucket on Cat 5130 B Hydraulic Excavator when he slipped on tracks & caught himself with his left arm. Causing Injury.
Fueling D11R Dozer when he slipped, the employee caught himself and strained his shoulder.
EE slipped off the hood at the front of the dozer, possible because of morning dew. His feet went out from under him and he thinks he hit everything he possible could on the way down. He ended up curled under the dozer. He got up before anyone got to him and crawled out from underneath the dozer.
There was rain that froze, the job site was slippery. The employee was leaving his equipment going to his vehicle to go home when he fell on ice and broke his ankle.
Inj EE & another EE were changing eng oil in D11N dozer. Due to weather condition, equip was very slippery. Inj EE lost his footing on catwalk, holding on to hand rail at door post, body weight jerked hand loose, pulling & straining rt shoulder & fell onto track, push arm and onto ground. Not sure which hit caused lower back injury.
EE was driving Euclid 85 Ton truck up the ramp to the dump area. Conditions of the ramp had been changing through the day. Truck lost traction & started sliding backwards. EE turned truck into bank on driver's side to avoid hitting another truck. Truck turned over. EE climbed out of the cab & jumped 10' to ground. He slipped, landing on his right shoulder resulting in a fracture.
Heavy equipment operator was walking the hill on the job to see where they wanted to make the next cut with the equipment. When they were going down the hill the injured employee caught his toe in a root & fell on his face.
EE'S WERE WALKING BACK TO THEIR CARS AT QUITTINGTIME. THEY HAD JUST CROSSED A COLLECTION DITCH &STARTED DOWN A SMALL EMBANKMENT WHEN INJURED TRIPPED AND FELL. HIS LT. LEG BUCKLED UNDER HIM, CA USING THE LIGAMENTS AND/OR MUSCLES IN HIS LT. THIGH TO TEAR.
EE AND THE MECHANIC WAS CHANGING OIL IN THE D11 DOZER. THEY WERE BOTH ON AND OFF THE DOZER SEVERAL TIMES. AS HE STEPPED OFF THE DOZER HIS FOOT SLIPPED OFF A ROCK, JAMMING HIS TOES INTO THE GRO UND, AND CAUSING HIM TO FALL.
