EMPLOYEE WAS USING A 3/4" BREAKER BAR AND "CHEATER" PIPE TO LOOSEN NUT ON 6 INCH WATER PUMP. HE PLACED FOOT ON PIPE TO EXERT MORE TORQUE. HIS FOOT SLIPPED OFF PIPE ALLOWING THE PIPE TO SLIDE O FF END OF WRENCH AND TRAVEL UPWARD STRIKING HIM ON THE CHEEK.
