At approximately 5:30 AM. Miner parked in the employee parking lot before work shift started. They plugged in their personal vehicle also lifting their windshield wipers to prevent freezing. They stepped on an uneven surface, approximately ¼ inch or less in height of the paved parking lot, rolled their left ankle resulted in a sprain.

Showing all 15After a heavy three day rain & blizzard event maintenance was removing snow from parking lot. The miner was asked to move EE's car in order to better clear all the areas. The miner slipped on EE's prosthetic leg which resulting in a fall on ice striking EE's head and received a concussion and abrasion on the back of EE's head.
The miner was exiting cab of pickup 6. While turning left the miner's clothes snagged the driver seat while stepping out, in doing so miner twisted left ankle. Miner walked off and the next morning left ankle swollen. Miner informed supervisor at 4:30 AM on 4/25/2019. Resulting in a sprain.
Utility miner stepped over some 6" pipe in the field and slipped on some ice twisting left knee. EE was placed on restricted duty.
Mechanic was pulling an Air hose walking backwards, 11/16/17. Mechanic tripped and fell on their right shoulder got back up and went back to work. Over the weekend the other shoulder started to hurt and they reported it on 11/21/2017 employee went to see the doctor. (strain)
The miner was pushing a 6 inch pipe and stepped across a small berm onto uneven ground and rolled right ankle causing a sprain.
The miner stepped off the lowboy trailer and when EE got to the ground EE slipped and twisted EE's left ankle. This resulted in a sprain of unspecified ligament of the left ankle, EE was placed on restricted duty.
Miner was standing on an extension ladder his feet were on 5th rung approximately 5 to 6' off the ground. He was welding just above the ladder on the back of the kress truck. The ladder slipped out from under him. He sprained his Rt. wrist trying to catch him self. Miner stated he failed to tie off the ladder or use the bed platform. Miner was put on restricted duty.
Mechanic was working on the A/C on a Motor Grader 16M he was he was stepping down of the machine and twisted his left knee. The mechanic continued to work for an addtional 30 min before he noticed any pain in his left knee. He notified his was supervisor and was taken to the doctor office and was told he had a sprained left knee.
Employee was standing in a 19" high stack of ripper shanks to be repaired, using remote control for a overhead crane. He was stepping clear and miss-stepped falling to the ground. This resulted in back strain, bruising and small cuts to his right forearm. He struck a bearing cooler with his forearm. Resulting in four lost days.
EE stepped over a beam in the walkway; used to slide a shaft out. He stepped off the bottom step and caught his foot on the chain, his foot rolled it to the inside, He thought he had just twisted it. Sat 7:30am He called the Safety Dir and told him about it, bruising and swelling. Told him to see a Dr. fracture to Rt.fibula, Dr visit Mon soft boot and limit movement.
EMPLOYEE LOST HIS BALANCE WHILE PICKING UP BACKUP STRIPS FOR WELDERS, FALLING FORWARD ONTO HIS CHEST. HIS LEFT LOWER ARM FELL ACROSS PLYWOOD COVERING A CABLE ACCESS HOLE PUSHING THE WOOD OVER. HIS LOWER LEFT ARM FELL INTO THE HOLE. NO OTHER CUTS OR BRUISES.
EE WAS CARRYING CARDBOARD BUCKETS OF GEAR OIL FROM A CLEANUP OF SPILLED OIL FROM INSIDE THE DRAGLINE TO A DISPOSAL BARREL OUTSIDE. EE STATES HER FOOT SLIPPED ON THE TOP STEP OF A 3 STEP STAIRW AY & SPRAINED HER LEFT ANKLE. EE STATES IT WAS RAINING & THE BOTTOM OF HER BOOTS MAY HAVE BEEN WET CAUSING HER TO SLIP. SHE WAS WEARING RUBBER OVER SHOES AT THE TIME.
EMPLOYEE HAD COMPLETED CLEANING THE REAR CAB WINDOWS AND WAS STANDING ON A PLATFORM ACROSS THE REAR OF THE MACHINE. HE MISSED STEPPED, LOST HIS BALANCE AND HIS HAND SLIPPED FROM THE GRAB RAIL CAUSING HIM TO FALL TO THE GROUND RESULTING IN BRUISED RIBS AND A SMALL FRACTURE TO THE LEFT WRIST.
OPERATOR WAS CLEANING RIGHT SIDE OF CAB WINDOW, UPON COMPLETION SHE LOST HER BALANCE AND HER LEFT HAND CAME IN CONTACT WITH THE EXHAUST PIPE. EMPLOYEE BURNED 4 FINGERS ON BACK OF LEFT HAND.
