Process Maintenance Tech was walking on a road during a snowstorm then they slipped and when they landed, they ended up sitting on their right ankle. Update: EE did not miss any scheduled shifts nor given restricted duties. EE did not improve & requested to be re-evaluated. EE continued to work with no restrictions until 2-13-25 when surgery was performed to right ankle.

Maintenance Tech was reporting to work and slipped on ice which resulted in rolling right ankle.

Operator put valve on 2 inch water hose and turned on valve and pressure knocked employee off balance and fell and hit head and jammed finger.

Miner was walking from employee parking lot to report to shift and tripped and fell and caught fall with RT hand.

PSES Operator was washing floors with water. There was some flock material on the ground and the operator did not see it slipping and falling injuring EE's right knee and left wrist.

Wash Bay employee was pulling some hoses when they slipped on some ice causing them to fall and injure elbow.

Employee was on the raised portion of deck on haul truck opening door to get bags out, in doing so they rolled their ankle. Employee was evaluated by MRT and taken to be seen at workman's comp doctor where employee was diagnosed with a sprained ankle. Employee released full duty but has to wear an ankle brace as well as had 6 pt appointment's.

EE was walking on the sidewalk and slipped on ice, hurt their ankle.

Employee was stepping off a carbon truck when the employee missed or slipped on a rung of the ladder falling off the truck and landing on side. The employee suffered a nondisplaced fracture of the left hip. Employee was given restrictions and put off work.

Operator tripped on sidewalk at the Ironclad falling and hitting face on handrail. Dry conditions and was at the end of shift. Employee received sutures for laceration.

EE working in the weld shop twisted right knee when walking across floor. EE was seen at ER where they were given work restrictions and has missed shifts. EE had reported to others at the start of shift the knee was sore.

The haul truck operator was dismounting the truck at the fuel island. When the operator reached the bottom step of the ladder and stepped to the ground, the ladder was higher than anticipated. The operator strained the right leg upon impact. This leg was previously injured at home just days prior. The operator was placed on restrictions until the follow up appointment.

The KK employee was removing refractory from an autoclave while on a work platform. As the employee stepped down from the platform onto the floor of the equipment, the employee mistepped and sprained an ankle.

Employee fell backward due to muddy conditions on the ground, and landed on left arm.

While walking to hot change van, Employee slipped on ice striking head on ground.

Operator fell while dismounting equipment and fractured right ankle.

IP was conducting a post blast inspection in the 4200-212. IP was standing on top of the muckpile checking for misfires. As IP was walking down the muckpile, the IP fell forward resulting in laceration to the left wrist. One suture was needed to close the wound.

When the bus arrived to drop off the crew for shift, employee stepped off the bus and twisted ankle.

On 12/23/2018 employee slipped and fell on left side of body and was initially given full duty release. After additional evaluation of the employee's shoulder in April 2019, the Dr. determined surgery was required on employee's left shoulder. Surgery was performed on 4/22/2019

On 11/15/2018 the employee stepped out of the service truck and felt a pop in left knee. This injury became reportable on 01/18/2019 when the knee became painful and could not bend it without more pain. The Dr. put the employee off work.

While exiting the bed of a service truck the employee slipped on the step falling 18" to the ground landing on right side.

When the flotation operator stepped down off of the concrete landing at the base of the stairs in the mill 5 flotation building basement, they felt a pop in the upper left calf area. The operator tried to take a few steps and apply some pressure to the left leg that they felt the pop from but continued to feel some discomfort. The operator then sat down and called for assistance.

This injury became reportable on 7/28/2018 when ee received a sedentary return-to-work release. Employee was walking across backfill material on uneven ground to access the trash pump at the lower sump in the Silverstar pit and twisted right knee.

While walking in established walkway, employee stumbled and fell to the ground resulting in a laceration to the left knee. Employee received 9 sutures to close the wound.

Employee poured wax on the floor in the admin, turned around to grab the mop and slipped in the wax falling on their right side.

Employee was using a pallet jack when EE slipped and fell to same level. Initial chief complaint was EE's left ankle. Next day the chief complaint was the upper left shoulder. Employee self cared and never lost/missed any work days. Became reportable when EE went to the Physical Therapist on 4/9/18.

IE was in the process of performing a preshift inspection on the haul truck when IE walked through the park ditch and slipped and fell on backside. Later that day the IE complained of a sore shoulder and backside and was given a restricted duty release after being evaluated by medical provider.

In September of 2017 IE reported a left calf injury while mounting a 793 HT. At that time EE was diagnosed with cellulitis or a skin infection which was not attributed to the injury. The IE has continued to have pain and tightness in EE's calf and on 5/17/2018 was prescribed a treatment of PT for peroneal tendonitis. There is no additional event since the injury in 2017.

IE's had been cleaning a sump that had never been cleaned by Redi Services. They had moved a large piece of grating for access to the sump for their hard pipe. The access was approx. 6-8". IE stepped on the unsecured grating and fell 25' into a sump causing a laceration to right pinky finger requiring 7 stitches.

While descending stairs employee felt a pop in left knee. Employee has a strained muscle in the back of knee.

While standing in between the engine and frame doing alternator drive accessory drive repair on CAT 793D haul truck, the employee went to step down on belly pan to get closer to work and felt like left knee popped.

The employee was exiting the administration building through the back single basement door. EE turned to the left to prop open the door; As they rotated to the right to walk to the truck, their right boot caught on the grating. With the forward momentum and the boot caught, the employee fell forward landing on their right arm.

While tailing-in a 10' PQ quill rod, the bottom of the quill rod kicked back causing the Helper to fall backwards onto a rod sloop resulting in a cut to the injured employee's right hip.

Employee was stepping over an enclosed auger when ee felt discomfort in right knee. EE went to the doctor that same day and was diagnosed with a mild strain and given a full duty release. On 3/8 ee went back to the doctor, given the same diagnosis and a restricted duty release.

Slipped over pipe ee was working on and through out hand to break fall

Employee stepped down after replacing a hose and twisted employee's knee (employee felt a pop). The employee sought medical treatment 02/16/2017 and was given restricted duty.

Icy Conditions. Employee slipped and fell on ice.

Employee was walking across a hard packed icy parking lot and slipped falling on right side and back Employee sought treatment on 1/25/17 that identified a fracture to the 7th rib.

Employee tripped and fell while walking; falling to the ground. Employee sought medical treatment and was taken off work for one shift.

Employee was working in engine compartment of 793D Haul truck. While attempting to remove the intake boot the Employee momentarily lost balance. In an attempt to catch self, employee caught left hand pinky on unknown object on the engine.

While getting out of a service truck the employee felt pain in the right knee when the right foot made contact with the (flat) ground. The employee went to the doctor and was put on restricted duty.

While descending a ladder off the pipe truck the employee stepped on a rock and hyper-extended left knee.

Pulling off an old swellex bolt and as employee pulled the bolt took a step back to get balance and slipped and fell backwards.

Employee had a 16 penny nail in EE's right hand when EE heard EE's name called. When EE turned to see who had called EE, EE tripped over an air line and fell to the floor. The nail cut EE's left wrist requiring 7 stitches to close. Employee was released with no restrictions and returned to work.

Employee twisted leg and resulted in EE falling and hitting EE's knee on a rock. Causing contusion and bruising to knee.

Employee was returning from the scale house to the truck, which was parked on the scale. As EE turned to change directions, EE twisted right knee, injuring it.

EE was in the process of depressurizing a discharge line on a portable pump when a reducer on the line failed. Discharge line contacted the ee and the ee was knocked to the ground at which time some material entered ee's right eye. This resulted in a corneal abrasion and a subsequent sensitivity to light which did not allow the employee to return to work.

Operator was coming down from HT and EE's right foot slipped off the rung of the flexible ladder and EE fell to the ground, landing on EE's right foot.

While exiting EE's cab a driller felt a pop in EE's knee when EE's foot hit the ground.

Employee fell onto his left wrist getting out of a pickup truck. Employee did not seek medical treatment until 01/22/2016 where he was put in a wrist brace and given a full duty work release.
