Showing all 4Employee was climbing a ladder and somehow missed gripping the ladder with EE's right hand about 5 feet up. EE fell from ladder but landed on feet at ground level. EE's right foot actually landed on another employee's foot causing EE's to roll and break at the ankle area.
SLICK CONDITIONS (ICE ON GROUND). HAD PREVIOUS PAIN IN SHOULDER PRIOR TO SLIPPING. ALSO HAD BUILDUP OF BONE SPURS PRIOR TO INJURY.
EE WAS CLIMBING DOWN THE LADDER ON THE POWDER TRUCK. WHEN HE SLIPPED, HIS ARM WAS CUT. THE CUT WAS SO CLEAN THAT HE HADN'T REALIZED HE WAS CUT UNTIL SOMEONE POINTED IT OUT. IT STOPPED BLEEDING SHORTLY AFTERWARD.
