While hanging a cable up, he stepped on the first rung of a two step ladder and felt the ladder tip backwards causing him to fall onto the belt line hitting his head on the belt line rail. The fall caused a head wound requiring 8 staples, to the back of his head.

Had an initial injury date of 1/28/10, received treatment but was not classified as recordable at that time. No further activity until several months later(Sept 15, 2010)when he had surgery on shoulder. It was unclear whether this surgery was related to work injury or a pre-existing condition related to aging. Review of injury has determined that it is reportable at this time.

Employee slipped on a loose rock and fell to the ground injuring right knee going to change house at EOS. No medical treatment.. surgery 11/18/2009. Pre-existing knee injury in 2003. Not clear if the new injury was new -no MRI performed for April 2009 event. Post-Op notes received on December 30, 2009 noted medial meniscus torn during the April incident.

Moving 6" vacuum hose to clean out the screw pit of ash. While holding/moving the hose with both hands employee lost balance & fell landing on left shoulder. This 70001 is being filed at this time because it has just now been determined that it was a work related injury.

Mechanic climbed up & was standing on a 3 1/2" angle iron. He was putting a chocker around a counterweight in order to lift it up & change the take up roller. After chocker was in place, he was attempting to get down, when he lost his footing, fell & landed on his back on the grating below.

While checking a water leak outside of a building the employee slipped on the ice, striking his left hand, hip and straining his elbow. The injury did not become recordable until he missed a day, November 18th. FMC was not notified that this absence was due to the injury until November 21. At this time the injury is just "pain" but tests are still being performed.

Employee caught his foot in the edge of the Admix Sump Pump. The sump hole had a small submersible pump that was barely keeping up with the inflow of water which caused a layer of water/mud to hide the adjusted grating. Employee's left foot slipped in hole; his momentum kept driving him forward twisting his ankle and applying pressure in the lower shin area.

Employee had washed an area down and then gone to lunch. Over lunch the water had frozen and when he returned he slipped while pulling the hose out of the washdown ditch. This are was indoors but there was freezing air blowing through the ditch and an open door. The employee suffered a broken foot and ankle that required surgery.

Employee was stepping backwards off the middle step of a 2' ladder. He stepped on a 13"x24"x7" rock that was slanted causing him to twist and fall to the ground. Injury became recordable when he had surgery on 1/10/05, however FMC did not learn of this until 1/26/05. The left knee injury was a torn meniscus.

The employe was on top of the tailgate drive cleaning off rocks. He started to get down, reached for a hose to hold on to, his toe hung up causing him to loose his balance miss the hose, falling head first into the pontoon on the shield. The tailgate drive is about 4 feet off the grouond. He required 8 stitches above his right eye and fractured the tip of his left thumb.

Employee was working on P-348; as he was walking aroung P-219 to get to P-348, he slipped on some ice and hit his left knee on the ground. This did not become a recordable until 3/16, when the employee went off to have surgical repairs to the knee.

EE WAS TAKING AN 8 FOOT 2 INCH HOSE FROM THE YARD CREW STORAGE SHED. HE PUT THE HOSE ON HIS SHOULDER AND STARTED OUT OF THE SHED. ONE END OF THE HOSE THAT WAS BEHIND HIM GOT HUNG UP AS HE EXIT ED THE DOOR. HE PULLED HARD ON THE HOSE TO FREE IT. WHEN THE HOSE BROKE FREE, HE LOST HIS BALANCE AND FELL HITTING HIS ARM ON SIDE BOARDS OF TRAILER.
