EE got out of EE's loader around 2:30 am near the surge pile ramp to stretch EE's legs, while walking around EE heard a loud screeching sound from the #2 belt. EE started to run towards the plant and twisted EE's ankle and fell to the ground. EE contacted another EE to let them know what happened. At 7:45 am EE was in pain and noticed swelling around EE's ankle.

While driving truck #2630 hauling production feed to the jaw, EE noticed a stone on the hood of the truck EE thought should be removed. EE stopped the truck and stepped out onto the second from the top rung on the access ladder to remove the rock. As EE was positioning self EE's hand slipped and EE fell backwards to the ground.

While descending stairs, on bottom 2 steps, fell forward bruising knee and scraping right forearm/elbow. Injury was not reported and no medical treatment was given. 9 days later, arm became infected and required hospitalization to heal infection.

Employee was getting out of skid steer, slipped on step and fell fracturing his left forearm. Ground was muddy.

Employee was replacing drive belts on transfer conveyor. When pulled the belt, it broke causing employee to lose balance and hit elbow on stacker then fell 3-4' to concrete pad.

While walking down wet concrete slope, employee slipped and fell back hitting head on concrete. Hard hat came off when employee fell. 1 stitch was required to close cut.

EMPLOYEE WAS WALKING ACROSS LOOSE STONES & SLIPPED ON ROCKS. HE FELL ONTO HIS BACK PUNCTURING ITON THE ROCKS WHEN HE FELL. PUNCTURE REQUIRED STITCHES.
