A mechanic was working on a fender of a belly dump trailer. While using a sledgehammer to straighten the fender metal the employee lost EE's footing which caused EE to slip and fall to the ground resulting in a fractured hip.

Employee parked EE's equipment and was walking to EE's light duty truck. Employee tripped and fell landing on EE's right hand. Employee was taken to the doctor for evaluation and X-ray. Employee was diagnosed with an Acute nondisplaced transverse distal radial metaphyseal fracture.

While walking EE stepped on some loose gravel and lost balance and fell on a knee. EE landed on a sharp rock that lacerated the right knee below the knee cap. EE received three stitches at the hospital.

Employee, returning from medical treatment without notifying company that EE had been injured on a previous date informed company that EE had a bulged disk in EE's back. The employee stated after leaving work EE slipped. The employee said EE didn't fall down or hurt self. After waking up EE felt pain in one of EE's legs.

Employee was looking over the area that was being drilled for blasting. While walking the outside berm the employee slipped and fell onto the berm resulting in a laceration of the right forearm.
