Showing 50 of 83Employee was exiting the loader, and EE's foot missed the last step on the ladder. Employee fell to mine floor, landing on EE's back. Initial medical evaluation was for a sprain/strain, but a follow up appointment on 12/16/2025 with X-rays determined EE sustained a broken rib.

On June 2, 2025, an employee was assisting with the installation of a packer intended to control a high-volume, high-pressure water leak at the mine face. During the operation, the employee began stepping back and turning, the employee twisted EE's right knee.
Temporary employee was working in the bagging area when EE tripped over a piece of the exit conveyor at approximately 1:30 pm. EE landed on EE's right shoulder and was experiencing tingling/numbness, but no pain. Employee was taken to the ER to be medically evaluated. EE did not attend EE's follow-up appointment, as required, and did not return or call into work until 5/19/2025.
Operator was stepping down off of a ladder after closing a valve when EE rolled their right ankle.
Employee's shoe became untied and employee tripped as a result. Upon hitting the ground during the call, the employee's glasses broke and cut employee's right eyebrow.
On December 18th, 2024, at about 11:30 pm the employee parked loader in fines shed adjacent to hopper 2. Employee was disembarking loader to clear metal detector fault. On reaching the ground the employee felt a pain above EE's right knee.
IE was rolling the tarp off of McNay rental trailer #118 when the handle detached from the trailer causing the IE to fall to the ground, same level injuring left lower back. The IE did not seek medical attention until 11/15, when they were taken off work.
THE IE TRIPPED OVER A PLASTIC BAND AND FELL STRIKING EE'S LEFT HAND AGAINST A PIECE OF METAL STRUCTURE. THE IE WAS TAKEN TO BLESSING HOSPITAL ER WHERE AFTER X-RAYS EE HAD A FRACTURE TO EE'S LEFT MIDDLE FINGER.
While dismounting the track mobile, IP stepped onto uneven surface with loose rock. The IP rolled their right ankle. IP was taken to Minor ER and diagnosed with a sprained ankle.
Employee was packaging material. Employee turned around to leave area and tripped on pallet lift table, fell and hit head. Diagnosed as possible mild concussion. No overnight observation.
Employee was climbing off of ladder. Employee missed a rung and fell approximately 3 feet to the ground landing on their rear end. Employee sprained knee sometime during the fall.
Employee was loading tanker truck. It was rainy and wet. Employee slipped and fell on top of the trailer. Employee tried to catch themselves with their hand and strained wrist. Employee did not fall off of truck.
On 8/3/2022 The Process Operator was working at Unit 3 operating the auto packaging system. At 2:40pm EE went to the bathroom at unit 3, upon leaving the bathroom EE missed the step and rolled EE's ankle. The Supervisor went and picked EE up and took EE to the minor emergency room. They did x rays and determined it was a sprained ankle with no fractures.
Employee tripped on uneven surface in mine and strained knee.
Operator was walking down a flight of stairs and sprained right knee. EE was not carrying anything, rushing and was using the handrail. EE was treated with prescription medication and physical therapy and given work restrictions.
IE was working on the crusher jaw at Mill 31. The IE was in the process of tightening bolts while working from the 3rd step of a portable ladder when the ladder slid out from under EE causing EE to fall to the concrete below. The injured suffered 3 fractured ribs.
Employee was walking down stairs in plant. Employee missed the last step and stepped onto landing at base of stairs. Employee felt pain in left knee. Both the stairs and landing were free from trip hazards and debris. Employee stated: just was not paying attention.
Facility had rain after several days and floors were wet causing the employee to slip and bend knee backwards.
As an employee was dismounting a haul truck, the employee's foot slipped off the bottom rung causing them to make contact with the ground. The employees hard hat fell off during the fall causing a minor abrasion to the back of their head. The employee was given medical treatment to clean the wound, and a prescription muscle relaxer and pain medication.
Employee exiting the haul truck rolled right ankle, put too much weight on right ankle when employee stepped on the ground.
Driver had stop at training and on the way back to truck the driver stepped off the edge of the sidewalk and turned ankle
Employee was climbing in drill when foot slipped off ladder and pulled shoulder out of socket.
Employee was stepping out of rail car mover and when EE stepped onto the ground EE twisted EE's ankle. Went to be seen at clinic. X-rays were taken and EE was diagnosed with a slight sprain. Missed two shifts then back to work normally. Ground was level and no work conditions were noted as cause.
Employee slipped and fractured left ankle.
Employee reported that they had slipped on flat, dry concrete causing an ankle to turn. The employee was treated at a local clinic for a pulled muscle and returned to work with restrictions. Company policy prevents employees working with restrictions, so employee was sent home until they receive a full release from the doctor.
Employee felt a twinge in back upon dismounting a forklift.
Shipping operator reported knee gave out while climbing down Silo 23 gantry stairs after moving a truck from silo farm.
Bulk bag fell over while employee was placing strap onto forks of fork truck. Employee lost balance and fell forward. Employee later in the day noticed back was painful.
Employee was getting out of EE's personal truck and when EE stepped onto the ground felt EE's hip pop. EE's truck is lifted and is very high off the ground. Employee was in awkward position and has had a previous hip surgery on that hip.

An operator was weighing a railcar, while an employee was spotting the railcar by track 6 and 8 during the early morning. The employee stepped an uneven ground, and twisted knee.

Employee was stepping off of a super sack cart and misjudged landing. EE twisted ankle resulting in a very mild sprain.

Employee started walking up a set of office steps. While on the 3rd step employee began to lose grip of coffee cup, by trying to catch it lost footing. Left foot slipped off the step and down onto the 2nd step hard rolling ankle.
Employee was getting into haul-truck 122, a CAT 769 rigid frame truck. EE opened the door, then turned around and was backing into the cab. When entering the cab EE's left foot either slipped or missed the walkway causing EE to fall to the left striking ribs on the left side on the handrail of a second truck parked next to it.
Employee was getting out of bobcat and foot slipped, causing employee to fall to the side of the unit. Right arm fell between two spare buckets next to the unit and was twisted, dislocating elbow.
Miner was in the process of cleaning fine material that had accumulated on a catwalk. During the removal of the material the employee stated that the maintenance access point failed causing employee to fall through the opening approximate distance of 8 feet.

Employee was descending platform steps and lost his footing between the second & first step. He used his hand to break his fall causing injury to his left hand. It was determined that he sustained a small fracture to his thumb.
Employee walked to retrieve a gladhand lock out of the lock bin on the shipping dock. Upon turning around to go load out a truck, the employee felt a pop in his right knee. Pain and swelling ensued soon after. Employee was sent for medical evaluation and assigned a 7-day work restriction.
Miner stepping down from step stool missed a step, causing him to fall backwards and strike back of hard hat on floor. Miner was sent to emergency room for evaluation, diagnosed with contusion, minor head injury, and minor neck/back sprain. Was prescribed painkillers and given work restriction of three days.
Icy surfaces -Employee was walking from building to truck and fell and hit his head on ground.
mill operator was finishing poking down the agstone in the outside system hopper. Operator got it all poked down and turned to put the poking rod up and slipped on some loose rock strained knee

EE was in the process of cleaning TK96/97 bulk scale with water hose when he stumbled and felt a slight muscle pull in his lower back. After task was completed he parked the water truck and started to exit the truck when he experienced pain in his lower back that restricted further movement. The process of twisting to exit truck caused the resulting sensation of pain.
Employee was climbing ladder at TK13 with a 45 degree angle pipe on shoulder when employee lost contact with 4th or 5th rung causing him to fall to ground level about 4 to 5 feet. Employee received a laceration to right arm at the elbow and minor abrasions to his back. Employee also received a minor cut to crown of head.
He explained that he fell while attaching the sack loops to the forklift. Access to forks on tow motor, and sack spout was being gained by a stack of 3 empty pallets, 4x4 boards, and a piece of plywood on top of the floor scale. As he stepped up onto the pallet configuration the plywood on top shifted causing him to fall.
