Employee was washing out the loader bucket with a fire hose. The hose became tangled and the employee was attempting to untangle the hose when he tripped over a roller frame that was holding one end of the hose down.

Employee was attempting to free a cable that was jammed in a winch. When the cable suddenly came free, the employee lost his footing and fell, striking right shoulder on the deck of the sand dredge.

EE WAS TIGHTENING THE BOLTS ON THE DRYER'S PINON BEARINGS. HE DESCENDED DOWN THE EXTENSION LADDER AND MISSED THE BOTTOM STEP. HE LANDED ON HIS LEFT ANKLE, SPRAINING THE LEFT ANKLE.
