Miner was sitting on the counter in test trailer and slipped while trying to get off. EE struck nose on the opposite counter. EE was taken to clinic for treatment.
Employee climbed up approximately 4' up side of conveyor to get a wrench and slipped, falling backward. Clothing got caught on the conveyor as they fell. A ladder was nearby, but not used.
Employee fell off trailer while adjusting a lifting chain on a screen that was being loaded on a flatbed trailer. Employee was taken to hospital for broken ribs and internal injury evaluation.
Miner tripped on a rock while shoveling. Miner landed on rock, striking lower back area. Miner could move on miners own, but was in pain and taken to urgent care. Currently unable to work until follow up appointment.

THE CRUSHING CREW WAS GETTING READY TO REPAIR A CONVEYOR BELT THAT WAS DAMAGED. TP DO THE REPAIR THE CRUSHING CREW HAD TO REPOSITION THE BELT. WHILE REPOSITONING THE BELT (EE IS A LOADER OPERA TOR) CAME ON THE SCENE AND ASSUMING THE BELT WAS POSITIONED & LOCKED OUT & READY FOR REPAIRS, GOT ON THE BELT TO ASSIST WITH REPAIRS. THE CRUSHER OPERATOR TURNED THE BELT ON, KNOCKED EE TO GR
EE WAS CLIMBING DOWN FROM CRUSHER WHEN HIS FOOT SLIPPED & HE FELL THE REMAINING DISTANCE TO THE GROUND. WHEN HE LANDED, HE TWISTED HIS RIGHT KNEE
