The employee was working on the bucket of a skid steer and while prying on the handles, the spry bar came loose causing the employee to lose balance and fall back. When the employee fell back EE caught self / fell on EE's ankle. The employee claims there were two pops and instant swelling.

The employee was switching pieces of equipment and was walking back to the shop when EE stepped on a piece of rubber belting that was covered in snow and rain. When EE stepped on it, it caused EE to slip and fall.

Employee was standing on the work platform of a service truck cutting angle iron with a cutting torch when EE lost balance and fell off the service truck. As EE was falling, EE tossed the cutting torch causing a few burns on the employees neck and back. When the employee fell, EE injured left foot/ankle.

Employee was climbing off a haul truck ladder when toe slipped off the bottom rung of ladder and the jerking of slipping pulled employee's arm and shoulder as employee was holding onto the handrail.
