Walking on wet scale walkway. Left leg slipped and twisted right leg/knee. Outboard side of scale walk is 80'x3' with 4' handrail. First 33' was covered with roll roofing for anti-slip. EE exited cab and was walking on non-covered surface.

The morning was cold/frosty and he was taking extra caution as he climbed out using the bumper as a step. Both feet were on the bumper, with one hand on tailgate. His right foot slipped, causing him to fall to the ground with right knee bent to side.
(OFFSITE/PRE-SHIFT) BECAUSE OF TRAVEL DISTANCE AND EARLY FIRE CLOSURES, WE WERE ALL STAYING IN OUR CAMP TRAILERS OFFSITE SO WE COULD START AT FIRST LIGHT. EE GOT UP AND WENT OUTSIDE IN THE DARK. EE STUMBLED ON UNEVEN GROUND AND FELL. HIS HEAD HIT A STAUB (BROKEN OFF LIMB) ON A SMALL LOG ALONG THE ROAD.
CREW WAS REHANGING PIECE OF SHEET STEEL ON CHAINS BY JAW TO KEEP ROCK FROM GOING ONTO BEL. EE WAS STANDING ON GUARD ON END OF BELT FROM CONE. FOOT SLIPPED ON ACCUMULATE MUD/ROCK. EE LOST BALAN CE & IMPACTED CHEST/UPPER ARM WHEN HE FELL TO GROUND.
EE WAS CLIMBING LADDER ON 980C CAT WHEEL LOADER. QUARRY FLOOR WAS WET/SLIPPERY. EE HAD BOTH HANDS ON LADDER RAILS AS HE WENT UP LADDER WHEN BOTH FEET SLIPPED. HE CAUGHT HIMSELF BUT HIS WEIGHT DROPPED ONTO HIS LEFT SHOULDER.
