EE was exiting heavy equipment (rock breaker Komatsu 200) at the end of his shift on Tuesday May 22nd around 6:30p.m. EE stepped from the tread of the equipment onto the step when the step broke causing the EE to contact the tread/broken step with his lower right leg. EE did not report injury to supervision until Wednesday May 30th.

Showing all 3Primary crusher truck dump bridged and had to throw chain under/over large rocks to break bridge. Employee proceeded to climb into the hopper to break the jam, while doing so employee stood on a rock instead of a flat surface. The rock slipped out from under the employee which caused the employee to fall striking his tailbone on a rock. Employee suffered a bruised tailbone.
Employee was looking for a survey marker on a joining lease property fence line, when he tripped on a small rock causing him to fall and twist his left knee.
