Operator was cleaning the operator cab windows from the drill deck, upon re-entering the cab the operator slipped on mud on the deck surface and fell at level impacting their right knee on the floor of the cab causing discomfort.
The operator was climbing the stairs to the haul truck and tripped over the second to last step from the top. The operator reached their hands out to catch themselves on the deck and upon impact hurt their shoulder.
Operator was in the process of clearing the OLD 101 Belt under the Secondary crusher area from a product plug. As EE was walking around the area next to the Old 101 Belt EE slipped fell in Mud due to previous rain, landing wrong on EE's right ankle
Operator disconnected the winch cable from the roll off dumpster. Then, the operators right foot/boot/ice cleat caught on the corner of the dumpsters frame causing them to lose their balance, tripping backwards and landing on their elbow/ shoulder straining their shoulder.
Operator was trying to pull the fuel hose to the haul truck. The fuel hose was caught on something, so the operator walked over to unhook the fuel hose and tried to pull the hose again, this time falling and landing on the right hand, injuring the wrist.
IP stated that they were at the mine sampling and due to the wet conditions, they lost their balance and fell down. The IP continued working throughout the shift and then advised another employee later in the day that their ankle was sore and went home. Seen at clinic on 10/16/2025.
Operator was descending a ladder to inspect underneath the loader, the operator experienced a sudden failure in EE's right knee as EE stepped down with EE's left leg. This caused EE to lose EE's grip on the handrails and fall approximately 3 to 4 feet to the ground. EE landed on EE's back on a youth nerf football-sized rock located on the north side of the loader.
At approximately 04:30 am, Craftsman was walking from change house to truck shop through haul truck access road walkway. As they approached eastern side, they stepped on a rock and was knocked off balance, but not falling. The felt a crack/pop in lower back. The pain increased later in the shift. Supervisor was notified at 10:30am and then taken to clinic for further evaluation.
Worker was walking to a hole they were about to measure when they tripped and rolled their ankle on uneven ground. Worker was assessed by EMT at 10:21am. EMT provided worker with an ice pack and advised them to go Medical Clinic for further evaluation. Supervisor took worker to MC.
At approximately 18:18 after pre-shift meeting, an operator was exiting the lineout tent on their way to the tie-line buses. Then, the operator caught EE's left leg on the boot scrubber that was being used as a door stop, causing the operator to fall and land on EE's left knee.
While going down the stairs of the haul truck, the operator's bag swung in front of operator causing operator to lose balance. Operator used left hand to help catch self and brace self from falling down the stairs and heard a "pop" in elbow. Did not call for a supervisor and went home. Pain became worse so operator went to an urgent care.
2/3 EE got down from loader to 3' ladder, EE's pantleg caught on a bolt, EE lost balance, fell, and twisted knee. 2/4 EE taken for a medical eval. and cleared to return w/o restrictions. EE follow up 2/24, 3/28 still cleared to return w/o restrictions. 4/21 MRI shows possible strain/tear of right knee ACL PCL. 5/30 EE referred to PT and cleared to return with modified duty.
At approx. 0730 on 01/14/2025 employee was walking back away from a scrap pile of steel and slipped into a tire rut which was iced over. Employee landed on left side/shoulder. The employee was provided with restrictions and able to return to work adhering to restrictions.
12.26.24 Worker was walking on the BCT tieline going past IP's haul truck when IP slipped. IP was wearing ice cleats. IP landed on backpack and right hand. Right hand and wrist became swollen, which radiated into forearm. On 01.09.24 worker was provided with restrictions since recovery was not ideal. (Submitted initially on 1.14.25. was voided. Resubmitted 1.16.25)
On 11/2/2024, the operator of Haul Truck 452 slipped and fell headfirst down the stairs during shift change, sustaining injuries. A neck injury remains an issue, leading to work restrictions effective 11/25.
Employee (EE) was delivering 16' jumbo steel to the 3520 ORD9 using an Integrated Tool Carrier (IT). Once parked, EE offloaded two pieces of steel. While lifting both pieces of steel under their right shoulder, EE began walking forward. As the steel came off the IT, the imbalanced weight caused EE to fall to ground. As EE was drug to the ground, pain was felt in the right ankle.
At approximately 7:25 AM, an analyst slipped and fell after getting wrapped up in a plastic tie used to secure clean towels used to keep the lab countertops clean and equipment dried. The employee suffered a fractured left patella.
EE was climbing down a 5-foot ladder to gain access to a guard. Their right foot slipped off the second rung from the top, causing EE to fall and slip off the ladder, landing on the right ankle. This caused a smell fracture to their heel.
Employee tripped and fell when crossing the haul road in front of the truck shop. They stopped their fall with their left wrist and the left elbow made contact with the rib cage. The following day severe pain was experienced and medical help sought. Diagnosis of 2 fractured ribs, and contusion of thorax.
The IP carried the drill hammer from the pickup truck to the support truck. As the IP climbed onto the fourth step EE's foot slipped and started to lose balance. With the hammer's weight, the IP fell forward and dropped the hammer which landed on the IP's fingertip.
Employee was standing on top of a pallet grabbing boxes off the back of the pallet. Their foot got entangled in some plastic wrap around the pallet causing a fall. The employee landed on their right wrist. Employee was seen at emergency room, diagnosed with acute fracture of distal radius requiring surgical repair.
Worker slipped due to the muddy conditions and hit forehead on the end of a steel bar as EE fell. Deep laceration required stitches at a clinic.
Operator slips on ice at tie line and receives partial tear to left quadricep tendon.
The employee was exiting an 80 ton crane while holding onto the provided handrails. The employee planted their right foot on the top platform just outside the cab and started to bring the left leg to the step when the right foot slipped causing them to lose their footing. The employee maintained a grip on the left cab handrail which prevented them from further falling to the ground
Employee was setting up to perform a vacuum job. The area had a slip hazard from flocculant, so the employees were attempting to clear the area. Employee slipped and attempted to brace self, injuring EE's right wrist. After being examined at via x-ray, employee suffered a fracture.
The employee slipped while performing cleanup and sustained a laceration to the right shin, the wound needed stitches.
While trying to move a table EE slipped and fell backwards twisting left ankle when falling to the ground. Employee suffered a fractured ankle.
Employee was descending a set of stairs outside the mudbird control room when left foot slipped on ice causing it to slip out from under the employee. The employee's right foot twisted when falling forward. The employee suffered fracture to right leg.
A maintenance craftsman was climbing onto the boarding ladder of an electric shovel when they felt a pop in their right foot. The employee was seen at a medical facility and given restricted duty pending a follow up appointment.
Employee was walking to truck and tripped on a section of raised sidewalk. As a result, they fell forward, landing on knees and hands.
While climbing down the boarding ladder on a Komatsu 930E haul truck the operator stepped to the ground and felt a pain in leg. Upon medical evaluation it was determined that they had a torn calf muscle. The ground surface was clean dry and flat.
Employee was working on rebar mat, material fell through mat employee fell approximately 3 feet contacting chest causing 3 rib fractures.
Employee slipped striking face on the handrail causing injuries.
EE was checking instrumentation when they tripped over a wire cable that is holding the temporary pump lines to the pond's leakage containment (LCRS) system. The EE fell forward onto the concrete with their hands out in front of them.
Employee's foot was stuck in mud / material, EE started to fall and let off the blaster hose that EE was spraying causing EE's momentum to go forward causing EE to fall.
An Operator was hooking up a hot soft water hose to the discharge line of the Gribben Sump to verify if line was clear or plugged. The Operator went to step up on the grating about 8 inches high and 36 inches wide and caught the toe of their boot on the overhang of the grating causing the Operator to trip and land on their right knee resulting in injury.
Employee fell from a standing position to a sitting position. Employee was removing bolts from the transition off the radiator cover of a front-end loader and the side employee was standing on fell approximately 8 inches, knocking employee down causing pain in the tailbone area.
Employee tripped over edge of a piece of fire blanket that had been dislodged by another contractor and fell onto EE's knee. EE has been diagnosed with a hematoma and potential strain. No medical or work restrictions.
During the pre-shift inspection, an operator stepped between the tires of the haul truck & the berm. When they placed their left foot on the berm it made contact with mud causing the operator to fall on their shoulder. At 2:30pm, the operator reported the injury. The operator was seen by a physician & given restricted duty for 5 calendar days. Employee has returned to full duty.
Miner was performing an engine repair on a CAT 794 haul truck. EE was coming out of the compartment to a shop ladder that was placed between the haul truck tire and the permanent ladder on the haul truck. EE lost footing and fell landing in a way that dislocated and fractured the ankle.
Employee was standing at EE's desk while making a phone call and fainted, falling and striking the right side of EE's head. Employee explained after that EE had recently been experiencing migraines.
Two operators were at the east side of the cold crystallizer to drain it. Operator climbed the ladder to put a water line on the draw off. Operator climbed down the ladder and lost grip from the ladder and fell backwards hitting the ground.
Employee was working inside the building on a sloped concrete floor. EE had tied the ladder to the structure and was in the process of welding a flange when the ladder slightly shifted startling the welder. EE then jumped down from the third rung of the ladder and landed on the concrete floor hurting EE's right heel.
miner strained left bicep FA on 11/23/19 then on 11/28/19 slipped on ice FA, but told to follow up with specialist for MRI. On 12/18/19 I was informed MRI showed a tear that will require surgery
Employee closing a 24" valve with a wheel handle, awkward access and hard to turn. Employee put their body weight into the valve handle to help get it to turn. When the valve handle finally loosened and was able to turn, the employee lost their balance and fell. In the process, they wrapped their arm around the valve to break their fall, tearing the tendon in the bicep.
Officer was hurrying to ensure there was no unauthorized access at the gate. Stepped off curb, twisted ankle. Step is 7 inches tall. Boots were laced, but loose. Officer has sprained ankle.
Employee was walking down stairs. The boot caught a stair and injured the knee as the fall took place. 3 pts of contact was inplace
A trespasser gained access to the mine pit area. The trespasser was running around the pit, Emergency services contacted. Trespasser threw self off of two 50ft highwalls
