While checking Feeder Hopper, fell back and cut arm on expanded metal.
Employees were cleaning the receiving lip of a screen box. There was wet clay material on the punch plate screens within the screen box. While standing on this screen the employee slipped on the wet clay, fell backwards, and hit their head, causing a gash in the scalp. The gash was accompanied by a raised bump and swelling. The laceration was closed with 2 staples.
The miner was preparing to set crane cables to lift a conveyor. The conveyor was lying flat on the bed of a trailer. The miner attempted to set the crane hooks by standing on the trailer tire. The miner lost grip/footing and turned to jump to the ground. When landing, miner's right foot was supinated, causing a tibia fracture.
the employee was sitting on the load out bunker belt preparing to hang the remote. when EE pushed the button and sending EE off the belt. employee fell (Flew) 18 feet to ground. broken wrist, nose and chipped 2 teeth.
While walking on road bed material of 5/8" crushed rock, employee stepped on right foot wrong way and rolled right ankle. Went to Doctor on 31 Jan 2014 found to have fractured bone in right foot.
Employee was closing the rolls when he walked outside of the designated catwalk and stepped on to a belt guard that was slick causing him to fall 12 feet to the ground. He broke his left leg and his nose when he hit the ground.
Slipped on a muddy surface causing a fall. Employee landed on a piece of steel tubing, with the ribcage taking the brunt of the fall. Employee diagnosed with two broken ribs.
Doing maintenance on Thor Stacker conveyor, putting wear iron in receiving box using torch and welder. He slipped and fell because of rainy conditions and smashed and cut his middle and pointer fingers on his left hand.
Dust on the hood of the machine made for a slick surface and the EE slipped and fell approx. 4 feet hitting right side of his back/ribs on the guard to the central panel while landing on his feet causing severe pain in the rib area.

EE was installing a wipe ont he belt press. He was standing on a conveyor in front of press. He pulled on a rope to move lever. The rope came untired and he fell to the slab below 6' He did not wear fall protection.
This is on injury that developed after a non-reportable fall 11-16-04 where the person caught his right foot in a tail pulley handle guard when he slipped and fell and at that time did not have injury and went back to work. his right foot toe dislocated now.
EE was running the dozer on bench #4, he climbed the slope to talk to other ee, when he got to the edge he lost balance and fell down about 15', he landed on his hands, he cut the right hand thumb.
EE SAID WHEN HE WAS ON THE LADDER OPENING & CLOSING A VALVE TO TAKE MUD OUT OF A TANK, THE LOADER BUCKET TOUCHED THE LADDER AND HE FELL INSIDE THE LOADER BUCKET.
HE WAS WALKING AROUND THE FEEDER AND HE TWISTED HIS ANKLE.
WALKING TOWARD BUTTON BOARD STEPPED WRONG OR TRIPPED TWISTING LEFT KNEE. DID NOT FALL CAUGHT HIMSELF ON THE BUTTON BOARD
