While coming down from changing breathers on hyd tank, employees left foot slipped from bottom step of the ladder. He landed on the edge of his foot causing it to twist to the left. He was taken to the hospital and given a full duty release but on a follow up exam (05/06/08) it was discovered that he had soft tissue damage and was put on Restricted Duty.

While employee was walking ground gave way and employee fell backwards into a hole and left ankle was caught between two rocks and twisted
Employee was walking between core tables and to avoid some water she stepped over to the right side. The grate was sticking up a 1/2" above the others and her toe caught on the grate. She tried to catch herself as she was falling but she twisted her body and fell on her right side. She went to the doctor on 5/27/08 and this was when it became a Lost Time.

GIVEN PRESCRIPTION FOR PAIN PILLS AT ER; X-RAYS NEGATIVE. (DELAY IN FILING INJURY REPORT BECAUSE COMPANY THOUGHT ONLY 1X INJECTION FOR PAIN WAS GIVEN AT ER.)
MINER WAS GETTING OUT OF A LHD AND TWISTED LEFT ANKLE.
Cold weather, icy frozen ground conditions. Worker had both feet on the ground when she lost her footing, fell backwards to the ground striking her tailbone on the frozen surface.
Miner slipped on snow walking to # 2 shaft, fell down spraining right wrist.

This miner was getting off a tractor when his left ankle rolled.
Ee was walking to the pickup truck when he fell & injured his shoulder. Snow on the ground may have contributed to the fall. Ee can give no explanation for the fall & there are no witnesses.
Employee was putting away a tool in upper tool box on water truck. His foot slipped off the step, then his hand slipped off the hand rail and he fell about 3 feet to the ground. He put out his left hand to catch his fall and fractured his left wrist.

THE EE WAS WALKING TO THE SHAFT FROM THE OFFICE, WHEN HE SLIPPED ON SNOW & ICE THAT WAS ON THE ROAD. THE EE LANDED ON HIS BACK WHEN HE FELL. THE EE DID NOT SEEK MEDICAL ATTN. UNTIL MARCH 22,2004 & AGAIN ON APRIL 6, 2004.
EE WAS HELPING A MINER LIFT UP A PLUGGER AFTER THE MINER HAD FINISHED DRILLING A 4 FOOT HOLE. THE EE HAD HIS RIGHT FOOT ON A 2X4 SUPPORT THAT WAS LAYING ON THE 2 INCH SIDE. WHILE PULLING THE PLUGGER OUT OF THE HOLE THE EE'S FOOT SLIPPED OFF THE 2X4. THE EE STATED THAT HE FELT A POP IN HIS RIGHT KNEE.
INJURED EE SLIPPED WHILE DISMOUNTING UG HAUL TRUCK, STRIKING LEFT SIDE ON AIR CLEANER GUARD.

EE WAS PUMPING FUEL INTO TANKS ON BACK OF PICKUP. HOSE WAS TANGLED WHILE STRAIGHTENING HOSE, HE SLIPPED AND CUT LEFT SHIN ON SIDEBOARD OF PICKUP BOX, REQUIRED 3 STITCHES.
EE WAS GETTING OFF THE 451 TIGER, HIS FOOT SLIPPED OFF THE STEP AND LANDING ON HIS RIGHT KNEE HESTRAINED HIS KNEE.

EE WAS WATCHING LOW PRESSURE END OF PUMP DISCHARGE WHEN PLUG IN 4" LINE DISCHARGED, CAUSING 4" RUBBER HOSE TO STRIKE EE IN CHEST, KNOCKING HIM DOWN. HIT BACK OF HEAD ON ROCKS.
