Stepped in deep mud with right leg. Tried to move with left leg and it caused their right knee to hyperextend.
EE slipped and fell and cut EE's right forearm. It required 6 stitches and EE was placed on modified duty.
Tripped over ultra vac hose and used left arm to brace themselves. Felt a pop in shoulder.

Employee was walking and tripped. Fell on left shoulder and head on concrete surface. Diagnosis through emergency room visit: cervical sprain; contusion left shoulder, minor head injury (concussion). Employee will be off work at least two (2) days. No witnesses, undetermined what employee actually tripped on. Area in question as been reviewed and no obvious obstructions.
Walking and slipped in grinding aide liquid, hurt left ankle, 3 broken bones, immediate surgery
On final day of EE's job, employee was descending a 15' fixed ladder with a cage when EE's foot slipped and EE fell approximately 10'. Employee broke EE's left fibula.
Stepped wrong walking up stairs and landed hard on left foot. Seeked medical attention on 12/12/2024. Sprained left foot. Prescribed a boot and was put on Sedentary Work.

EE missed step descending stairs at truck load out, as EE stumbled EE held onto handrail, straining the right shoulder. EE reported injury, but did not request medical attention for 2 weeks, until discomfort continued. Physician prescribed anti-inflammatory medication, and lifting restrictions.

Two employees were inside quarry crusher fines chute cleaning built up material. A large piece of buildup struck the walkboard causing the employees to fall onto the material. One employee chipped bone in elbow as it struck the bin. The other employee was not injured.

Employee slipped while descending stairs from lab. Steps were wet, but had anti-slip material on steps. Employee had been experiencing pain from non-work related plantar fasciitis previous to incident, but stated that the slip caused an increase in pain. Physician prescribed limited use of foot.
