Showing 50 of 88Employee had to stretch to get a plastic cover over a super-sack; in doing so, the employee slipped and fell. Upon impact, the employee's left thumb was jammed into the side of the pallet. The employee was able to stand up but was unable to move EE's thumb without pain.
Employee was walking to locker room at the end of EE's shift to go home. Incident occurred by the Sweco/Duavac in Building #02. Employee was walking and felt a sharp pain in left knee and lower back making EE unable to walk on left leg.
IE was walking across yard outside Warehouse #1 and slipped on ice falling to ground. IE fractured left ring finger and bruised left shoulder and knee.
At roughly 4:30pm on July 22nd, 2024, an employee with a little over 6 years of mining experience tripped on the 2nd step as the EE was ascending the stairs in Shed 8. EE fell on the stairs injuring knee, shortly after which the employee left work and went to the doctors where EE was diagnosed with a contusion on knee.
Employee was kneeling on top of a double stacked pallet, forklift driver was down stacking pallets to be identified by the tag on the ears. Employee kneeling on the double stacked pallet on the next row reached for the ear and fell forward finger was damaged on the fork.
Tripped on beam lying beside tank outside building by ball mills and hurt knee and then back hurt the day after
was performing work in the forklift and knee buckled.

Employee was complaining about legs hurting, supervisor then brought EE into office to cool down. After resting for a few minutes EE was still complaining about pain in the legs. Management decided to take EE to BHP to be checked out. To go shower out, EE fell down the stairs. Supervisor ran out to check on EE and called for management to call ambulance.
Employee was working around the sack hanger when they missed a step coming down the stairs causing them to fall and land on their bottom.
Employee slipped and fell in employee parking lot. Monday snowstorm had left ice accumulations that were treated and continued to be addressed as temperatures were fluctuating. Parking lot was plowed and treated with salt on Monday and retreated as the week went on. Employee has strained back and return to work date on 1/24/22.
Employee walking beside railroad track tripped on rope tied to guardrail. EE stumbled but caught balance. EE reported no injury at that time and was able to finish the rest of the shift. After a day or so EE's foot started swelling so we sent EE for X-Ray.

The employer was notified of additional restriction on 8/17/20. This accident sent 8/17/20 and failed to submit documentation of the attempted submission exists.

Employee was dismounting a forklift when EE's foot slipped. Employee landed on left leg and experienced pain in the left knee. Employee was sent to hospital for evaluation and is to return to full duty on 2/17/20.
By compactor, employee was pulling a box full of junk, box ripped and employee fell backwards. When Employee fell backwards employee jammed left wrist. Employee was given a velcro splint to wear for two weeks.
Employee was walking to reset 10 scale in packing room when EE tripped onto a pallet of cardboard. The employee fell onto the cardboard on EE's right side and arm.
Employee was helping to move a band saw when it tilted. The employee fell in attempt to get out of the way, landing on EE's left elbow and shoulder. EE did not seek treatment at the time. EE sought treatment on 7/10/19 and was sent for therapy. Company official who reports was notified 7/31/19.

Employee was descending steps from a loader and slipped. The employee caught self using the hand rail, but strained back.
Employee had purchased new safety shoes. EE was descending a set of concrete stairs and slipped but did not fall. EE had a small scrape on forearm. It was not reported as an accident at the time until the employee began complaining about back pain. The employee stated ee did not know if the slip had caused the issue.
Coming down spiral staircase thought I was down but missed last step and fell on floor my radio was on R front pocket.
Employee was unlocking and opening the gate to the truck scale. Employee lost hold on the gate and tried to grab it so it would not swing out toward the roadway. In employee's attempt to grab the gate employee overturned ankle causing a sprain.
EMPLOYEE WAS STACKING BAGS ON PALLET. EE PLACED A BAG, TURNED TO RETURN TO THE BAGGER AND CAUGHT FOOT ON THE CORNER OF THE PALLET TWISTING RIGHT KNEE.
Outside yard forklift operator was dismounting forklift and stepped on a 1.5" rubber ball (debris)(used for tumbler screening) which caused EE to slip and fall injuring shoulder and back. One doctor released EE back to work full duty for the shoulder injury. A different doctor is keeping EE out of work for the back injury.
Worker slipped on a stairwell due to wet and graphite slicked boots from rain and surface dust on staircase. Worker had bruising, pain and mobility issues at the time of accident and requested to see a doctor for evaluation. The doctor put the worker on light duty and recommended rest so the worker decided to take a few days vacation and was released for full duty on 3/6/2016.
EE was walking from the employee parking lot to the main property when EE slipped & fell on the ice injuring EE's back. We had previously had over 20 inches of snow over the weekend creating very slippery conditions. Maintenance had cleaned the lot earlier in the morning, but ice under the snow created a slippery condition. The area had been plowed & treated prior to the injury.
Employee was descending the stairway at the screen plant. It had previously rained and washed material and water to the bottom of the stairway. He did not notice this at the bottom of the stairway and slipped hyper-extending his left knee.

EE had loaded the 928G front end loader onto a lowboy trailer. He got our of the loader and was climbing down the ladder. He missed the last step and fell backwards onto the ground landing on his shoulders and hitting the back of his head on the pavement.
