P & P Materials LLC · Fall from ladders

Employee placed a 12' extension ladder on a 8" water pipe to remove a camera. The ladder was not extended and was tied off to the water pipe. After removal of the camera he was descending when the top of the ladder shifted. He lost his balance and fell approximately 6' to 7' landing on a concrete slab, injuring his left ankle.

EMPLOYEE PERFORMED THE PRE-OPERATION INSPECTION & CLEANED THE FRONT WINDOWS. AS HE WAS IN THE PROCESS OF DESCENDING THE LADDER ON THE RT.SIDE OF THE MACHINE, HIS HAND SLIPPED OFF THE HANDRAIL & HE FELL BACKWARD TO THE GROUND, LANDING ON HIS BACK. EE HAD A FLASHLIGHT IN ONE HAND, NOT ALLOWING COMPLIANCE W/INSTRUCTIONS GIVEN ACCORDING TO .11011- USE OF LADDERS.
